CREPEY OR DROPPED? · 1. SOFWAVE™ TIGHTENS · 2. NECK THREADS LIFT — DR NISH DECIDES

Non-Surgical Neck Lift London – Tighten with Sofwave™, Lift with PDO Neck Threads. By Dr Nish Shah MD, Mayfair

Necks age faster than faces. The skin is thinner, it has fewer oil glands, and it spends all day bent over a phone. By the mid-40s most necks have done one of two things, and some have done both. The skin has gone crepey — that is tightened with Sofwave™, which is FDA-cleared for the neck. Or the under-chin has dropped and folded — that is lifted with 720 PDO neck threads, and you see it before you leave. Dr Nish Shah MD performs both and decides which your neck needs at your assessment.

1 · Tightens — Sofwave™ Neck

– Crepey, lined, loose neck skin
– FDA-cleared for the neck and under-chin
– About 30 minutes; nothing to hide afterwards

2 · Lifts — 720 PDO Neck Threads

– An under-chin that has dropped and folded
– Repositioned the moment the threads go in
– Holds 12–18 months

1+2 · Both, staged — Threads, then Sofwave™

– Folded and crepey
– Lift first, tighten over the new line
– Spacing set at consultation

Is it fat rather than skin? Then it is treated as fat. Dr Nish checks by pinching before anything is planned.

Neck first, not face. The neck is assessed on its own: pinched, turned, tilted. Crepey, folded, fat, or a mix

Cleared for exactly this. Sofwave™ holds an FDA clearance to lift lax under-chin and neck tissue, not a face clearance stretched to the neck

A chin-to-neck angle you can see in profile. Neck threads reposition the fold; the angle comes back on the day

Told if it is beyond threads and ultrasound. A neck with real skin excess is referred for surgery, not booked for a treatment

Taught, not learned. Dr Nish trains other doctors in neck and jawline thread technique at Derma Medical

No stitches, no scars behind the ears. Sofwave™: back to work today. Threads: back tomorrow

The Treatment

Why Does the Neck Go First?

Neck skin is about half the thickness of cheek skin, has fewer oil glands, and sits over a thin sheet of muscle rather than bone. It also gets forgotten: sunscreen stops at the jaw, and the chin spends hours tipped down at a screen. So it loosens early and shows it quickly.

Two things happen, usually a decade apart. In the 30s and early 40s the skin goes crepey: fine horizontal lines, a texture like tissue paper when the chin is tucked. From the mid-40s the tissue under the chin loses its attachment and folds forward, blurring the angle between chin and neck. The first is a skin problem. The second is a position problem.

Crepey skin is tightened. A fold is lifted. Treating one with the other is why so many neck treatments do nothing. Dr Nish Shah MD examines the neck on its own — pinched, turned, in profile — and tells you which yours is. Sofwave™ for crepe. 720 PDO neck threads for a fold. A staged plan where there is both. And if the excess is beyond either, that is said before anything is booked.

CREPEY, FOLDED OR BOTH

The Neck Assessment — What Has Your Neck Done?

Three findings decide the plan. Dr Nish performs the treatment for each himself.

  • Crepey, still in place → Sofwave™ neck. Lines and tissue-paper texture, but the under-chin has not folded. Ultrasound warms the collagen layer 1.5 mm down and the skin rebuilds its own. About 30 minutes, no downtime, firming for 6 months. FDA-cleared to lift lax under-chin and neck tissue.
  • Folded under the chin → 720 PDO neck threads. The tissue has dropped forward and the chin-to-neck angle has gone. Dissolvable 720 Dimension COG threads draw it back and up, and the angle returns in the mirror before you stand. Holds 12–18 months, with new collagen laid along each thread.
  • Folded and crepey → threads first, Sofwave™ second. The usual picture from the late 40s. Threads reset the position; Sofwave™ then shrinks the skin to the new line. Sequenced by Dr Nish, spaced as the manufacturers advise, planned at consultation.
  • A pocket of fat → treated as fat. If the pinch test finds fat rather than skin, tightening will not change it. Dr Nish says so and plans fat treatment first, with tightening afterwards only if the skin needs it.
  • Real skin excess → a surgical referral. Where the neck has more skin than threads and ultrasound can take up, Dr Nish refers you for a surgical opinion. You are not booked for a treatment that cannot do the job.

Crepey vs Folded — Two Different Necks

Crepey — Sofwave™ Neck

— A skin problem

Looks like Horizontal lines; tissue-paper texture when the chin is tucked
Pinch test Thin, loose skin; no pocket
Treatment Sofwave™, one session, numbing cream
What you see Firming from 12 weeks; still improving at 6 months
Downtime None
Maintenance One session a year

Folded — 720 PDO Neck Threads

— A position problem

Looks like A fold or sag under the chin; the jaw-to-neck angle gone in profile
Pinch test Tissue that has dropped, not just skin
Treatment Neck threads, local anaesthetic, 30–45 minutes
What you see The angle back, on the day
Downtime 48 hours; 2 weeks of care
Maintenance Repeat at 12–18 months

WHICH NECK IS YOURS?

Crepey, Folded, Both — or Fat, or Excess?

Five findings, five answers. Dr Nish tells you which at the assessment, in the mirror and in profile.

Crepey — Sofwave™

  • Lines and texture, no fold
  • Usually 30s to mid-40s
  • One session, no downtime
  • Firms for 6 months

Folded — Neck Threads

  • Under-chin dropped forward
  • Usually mid-40s onward
  • Angle back on the day
  • Holds 12–18 months

Both — Staged

  • Folded and crepey
  • Threads first, Sofwave™ after
  • Sequenced by Dr Nish, to guidance
  • The common late-40s neck

Fat or Excess — Told Plainly

  • A fat pocket is treated as fat
  • Real skin excess is referred for surgery
  • Neither is booked for tightening
  • You leave knowing which you are

If you cannot tell which you are from the mirror, that is normal. The pinch test and the profile view at the assessment settle it.

WHAT WE TREAT

The Neck, Part by Part
— What Medisha Does

Each part of the neck is assessed on its own and treated with the answer that fits it.

Horizontal Neck Lines (“Tech Neck”)

Lines etched by years of looking down. Sofwave™ firms the skin they sit in; the lines soften as it tightens.

Crepey Skin Under the Chin

Tissue-paper texture with no fold. Sofwave™ is FDA-cleared for exactly this area. One session.

Under-Chin Fold (“Turkey Neck”)

The tissue has dropped forward. Neck threads reposition it on the day; Sofwave™ follows at a second stage where the skin is also crepey.

Blurred Jaw-to-Neck Angle

The line between chin and neck is what makes a neck read as young. Threads restore it; Sofwave™ keeps the skin tight along it.

Jawline Running Into the Neck

Where early jowls and a sagging under-chin meet. Jawline and neck threads together, then Sofwave™ neck.

Vertical Cords

The two cords that stand out when you tense are muscle, not skin, and are assessed separately. Threads and Sofwave™ do not treat them.

Suitability

Is My Neck Right for This?

If your neck has gone crepey, folded under the chin, or both, and the pinch test finds skin rather than fat, almost certainly. The assessment confirms which answer, or both.

Good Candidates

  • Fine horizontal lines and a crepey texture on the neck
  • A fold or sag under the chin that shows in profile
  • A jaw-to-neck angle that has softened
  • Early jowls running into the neck
  • A neck that has aged faster than the face
  • Wanting a visible change without stitches or a scar behind the ear
  • Any age, every skin tone

May Not Be Suitable

  • A neck with real skin excess — referred for a surgical opinion rather than booked
  • A pocket of fat under the chin as the main finding — treated as fat first
  • Prominent vertical cords as the main concern — assessed separately
  • Pregnancy or breastfeeding
  • A pacemaker or metal implant in the neck (Sofwave™)
  • Blood thinners that have not been reviewed (threads)

The pinch test and the profile view take 2 minutes, and they decide everything. Dr Nish does both at your assessment, £50, redeemable in full against treatment, and tells you which neck you have before any treatment is discussed.

Clinical Evidence

What the Research Shows

Sofwave™ is cleared for the neck specifically, and thread lifting has a published record of its own.

US FDA 510(k) K211483, November 2021

The FDA cleared Sofwave™ to improve facial lines and wrinkles, lift the eyebrow, and lift lax under-chin and neck tissue in adults aged 22 and over, the neck by name. The supporting study ran at 5 US sites with 80 treated patients and 467 treated areas: at 3 months, 80% of eyebrow patients and 85% of under-chin and neck patients were rated improved, with no serious adverse events. FDA 510(k) summary K211483, decision 15 November 2021

Journal of Cosmetic and Laser Therapy, 2018

Patients followed for 24 months after barbed PDO thread lifting showed lifting of 3–10 mm and high satisfaction, with a complication rate of about 4.8%. Ali, J Cosmet Laser Ther 2018;20(1):41–49

Dermatologic Surgery, 2021

In a prospective multicentre study of a single Sofwave™ treatment in 60 patients, 88% showed improvement on the investigator’s global assessment at 12 weeks and 86% improved on the Fitzpatrick wrinkle scale, with no device-related adverse events. Why Dr Nish Shah

Why Medisha for the Neck — Assessed Like a Neck, Not a Face

Most neck treatments are face treatments pointed downwards. At Medisha the neck is pinched, turned and viewed in profile before anything is planned, and the doctor who finds the answer is the one who performs it.

Crepey or Folded, Told at the Start

A 2-minute pinch test and profile view decide whether you need Sofwave™, threads or both. You hear the finding, not a sales pitch.

Non-Surgical Neck Lift FAQ
— Answered by Dr Nish Shah

Medically written and reviewed by Dr Nish Shah MD — Medical Director, Medisha Clinic · Senior International Clinical Trainer, Derma Medical · Official certified Sofwave™ practitioner and partner · Brand Ambassador & KOL for Intraline (PDO threads) · Last reviewed 9 October 2026

A non-surgical neck lift firms crepey neck skin and lifts a fold under the chin without stitches, scars or a general anaesthetic. Sofwave™ ultrasound is used for skin that has loosened; 720 PDO neck threads for tissue that has dropped. Sofwave™ is FDA-cleared to lift lax under-chin and neck tissue: it warms the collagen layer and the skin firms itself over 12 weeks. Neck threads draw the fold back on the day and hold for 12 to 18 months. At Medisha Clinic, Dr Nish Shah MD examines the neck on its own, pinched and in profile, and performs whichever your neck needs, or both as a staged plan.

Pinch the skin under your chin between finger and thumb. If it is thin and papery with nothing in it, it is loose skin. If it fills your fingers and moves as a pocket, it is fat. Many necks over 45 are some of both. Loose skin is tightened with Sofwave™ or, where it has folded, lifted with neck threads; a pocket of fat does not respond to either and is treated as fat first. Dr Nish does this test at the start of every neck assessment, because treating fat as skin is the commonest reason neck treatments fail.

A turkey neck is the fold of tissue under the chin that blurs the line between chin and neck. It is lifted with 720 PDO neck threads, which draw the fold back and up on the day and hold for 12 to 18 months. Where the skin over the fold has also gone crepey, Sofwave™ follows at a second stage to firm it over the new line. Where the fold is really a pocket of fat, it is treated as fat. Where there is more skin than threads can take up, Dr Nish refers you for a surgical opinion rather than book a treatment that will not change it.

At Medisha Clinic in Mayfair, Sofwave™ for the face and neck, which includes the under-chin, is £1,800, or £2,200 with the brow; a 720 PDO neck thread lift is £750, and jawline and neck threads together are £1,500. For a neck that has both folded and gone crepey, the Neck Lift Plan (neck threads, then Sofwave™) and the Jawline & Neck Lift Plan are sequenced by Dr Nish and quoted after consultation. Reviews are included. Consultation with Dr Nish is £50, redeemable in full against treatment. The clinic is at 58 South Molton Street, Mayfair, London W1K 5SL, 3 minutes from Bond Street station.

Yes — the neck and under-chin are two of the areas Sofwave™ is FDA-cleared for, under a clearance to lift lax submental and neck tissue granted on a five-site clinical study. It suits the neck well because the skin is thin and the energy is focused at 1.5 mm, above the fat, so a slim neck is firmed without being hollowed. One session of about 30 minutes under numbing cream, no downtime, firming visible at 12 weeks and still improving at 6 months. It does not move a fold that has dropped; that needs threads.

Yes, as a staged plan where clinically appropriate and in line with manufacturer guidance. Threads usually go first, to draw the fold back. Sofwave™ follows at the spacing set in your plan, so the skin tightens over the repositioned neck rather than the old one. Both stages are performed by Dr Nish and the order and dates are written down at consultation. This is the usual plan for a neck from the late 40s that has both folded and gone crepey.

After Sofwave™, straight away: there is mild redness for a few hours and nothing to hide. After neck threads, the under-chin may be mildly swollen or bruised for about 48 hours. Most patients are comfortable in a low neckline within a week. The 2-week care period is about avoiding massage, wide yawning and hard exercise, not about hiding anything. Thread entry points are behind the ear and under the chin and are not visible once healed.

If the angle has gone because the under-chin has folded forward, neck threads restore it on the day, because they draw the dropped tissue back and up rather than tightening the skin over it. If the angle has softened because the skin has loosened, Sofwave™ firms it over 12 weeks. If the angle was never there because the chin is small, that is a structure question, assessed separately. Dr Nish looks at the neck in profile at the assessment and tells you which of the three applies.

Horizontal neck lines, often called tech neck, are creases etched into thinning skin by years of tipping the chin down at a phone or screen. They are treated by firming the skin they sit in with Sofwave™, which softens them as the skin tightens over 12 weeks. They are a skin problem, not a position problem, so threads do not treat them. Daily SPF on the neck and keeping the screen at eye level slow them from deepening again.

The two vertical cords that stand out when you tense your neck are bands of muscle, not loose skin, so neither Sofwave™ nor threads treats them. They are assessed separately at consultation, and Dr Nish will tell you what, if anything, is appropriate. A neck can have cords and a crepey or folded neck at the same time, and the skin and the fold are still treated as described on this page.

A neck with real skin excess, where there is more skin than threads and ultrasound can take up, is referred for a surgical opinion rather than treated. A pocket of fat as the main finding is treated as fat first. Prominent cords as the main concern are assessed separately. Sofwave™ is not used over a pacemaker or metal implant in the neck, threads need a review if you take blood thinners, and neither is performed in pregnancy or breastfeeding. There is no upper age limit and skin tone is not a restriction.

Because the neck is assessed as a neck, pinched and viewed in profile before anything is planned, and the doctor who finds the answer performs it. Dr Nish Shah MD trains other doctors in neck and jawline thread technique as Senior International Clinical Trainer at Derma Medical. He is an official Sofwave™ partner and has performed thousands of PDO thread lifts among over 30,000 non-surgical treatments. A fat pocket is treated as fat and real skin excess is referred, so no neck is booked for a treatment that cannot change it.
