A heavy brow and hooded eyes have two non-surgical answers, and one honest one. Sofwave™ firms brow skin that has thinned and slackened; the eyebrow is named in its FDA lifting clearance. PDO brow threads carry a brow tail that has slipped back up, and you watch it happen in the mirror. Where the fold is stretched upper-eyelid skin rather than a low brow, no ultrasound or thread removes it, and Dr Nish Shah MD says so. He performs both treatments himself and shows you which your brow needs at the assessment.
1 · Tightens — Sofwave™ Brow
– The skin over the brow has thinned and slackened
– Eyebrow named in the FDA lifting clearance
– One 20-minute session, no needles
2 · Lifts — PDO Brow Threads
– The brow tail has slipped onto the lid
– Repositioned while you hold the mirror
– In place for 12–18 months
? · Eyelid skin — An Honest Answer
– The brow sits where it should; the lid skin has stretched
– No ultrasound or thread removes skin
– You are told, and pointed to a surgical opinion
Which one is yours? Dr Nish lifts your brow with one finger and shows you in the mirror.
The finger test first. Dr Nish lifts your brow by hand and shows you whether the hood goes with it
Cleared for the eyebrow by name. The eyebrow is one of the areas Sofwave™ holds a specific FDA lifting clearance for
Watch the brow tail come up. Brow threads reposition it there and then, while you look in the mirror
Told if it is the eyelid. Stretched upper-lid skin needs a surgeon, not a thread, and Dr Nish says so at the assessment
Taught by the trainer. Dr Nish teaches thread lifting to other doctors and has placed thousands of PDO thread lifts; official certified Sofwave™ practitioner and partner
No cut, no scar, no general anaesthetic. Sofwave™ and back to your desk; threads with 2 days of settling

The Treatment
Why Does My Brow Look Heavy, and My Eyes Hooded?
Your brow rests on a sheet of skin and muscle. As that skin thins and slackens, the brow settles a few millimetres lower. As the deeper support gives, the brow tail slides down onto the lid. The eye looks smaller, the lid looks heavy, and eyeliner vanishes into the fold by mid-morning.
Sometimes the brow has not moved at all. The upper-eyelid skin itself has stretched. That is an eyelid problem, not a brow problem, and no ultrasound or thread takes skin away. Telling the two apart is the whole job: raising a brow that is already in place does not open the eye.
At your assessment Dr Nish lifts your brow with one finger and watches what the hood does. Goes with the brow? Then it is Sofwave™ or threads, depending on whether the skin has slackened or the brow has dropped. Stays put? Then it is lid skin, and he says so and points you towards a surgical opinion.
TWO ANSWERS, ONE HONEST ONE
Loosened, Dropped, or Eyelid Skin? What Does Your Brow Need?
One doctor, two tools and a mirror. Your brow decides which tool is used, not the equipment list.
- 1. Slackened → Sofwave™ brow. The brow tail sits a touch lower and the outer lid feels heavy by evening. Parallel-beam ultrasound warms the skin across the brow and temple at about 1.5 mm, where collagen is made. About 20 minutes, nothing to recover from, and the eyebrow is named in its FDA lifting clearance.
- 2. Dropped → 720 PDO brow threads. The tail of the brow rests on the bony rim or has slipped beneath it. Dissolvable 720 Dimension COG threads, placed from the hairline, carry it back up while you watch. By 6–9 months the threads have dissolved; the collagen left along their path holds the position for 12–18 months.
- + Muscle-held → anti-wrinkle brow lift. Where the frown and outer-brow muscles are dragging the brow down, a small dose releases them. The brow rises 1–2 mm over 2 weeks. 10 minutes; 3–4 months. Often added to either answer above.
- Dropped and slackened → staged. Threads first, to carry the brow tail back up. Then Sofwave™ to firm the skin across the new, higher brow. Threads first, then Sofwave™; Dr Nish sets the order and the timing at consultation, where clinically appropriate and in line with manufacturer guidance.
- Stretched lid skin → a surgical opinion. If the brow is already where it should be and the fold is eyelid skin, nothing non-surgical removes it. The operation is an upper blepharoplasty, and Dr Nish tells you so rather than treating around it. Where some brow drop sits alongside, that part can still be treated here.
Sofwave™ vs PDO Brow Threads
Sofwave™ Brow — Tightens
— For brow skin that has thinned and slackened
PDO Brow Threads — Lift
— For a brow tail that has dropped
WHICH ONE ARE YOU?
Loosened, Dropped, Muscle-Held
— or Eyelid Skin?
Four hooded eyes, four different conversations. Find yours.
Slackened — Sofwave™
- Brow tail a touch low; skin thinner over it
- The hood lifts when you raise the brow by hand
- One 20-minute session; back to your day
- Eyebrow named in the FDA clearance
Dropped — PDO Brow Threads
- Brow tail resting on or under the bone
- Eyeliner lost in the fold
- Up before you leave; 12–18 months of hold
- Sofwave™ can follow, timed by Dr Nish
Muscle-Held — Anti-Wrinkle
- Frown muscles dragging the brow down
- A few small injections; 10 minutes
- Rises 1–2 mm over 2 weeks
- 3–4 months; often added to 1 or 2
Lid Skin — Surgery
- Brow in place; the hood stays when you lift it
- Stretched upper-lid skin
- Non-surgical may not achieve what surgery can
- You hear it here, before any treatment
Still unsure? Stand at the mirror and lift the outer brow with a fingertip. Hood gone: it is the brow. Hood still there: it is the lid. The assessment settles it either way.
YOUR OPTIONS
Every Treatment for a Heavy Brow
Under One Roof
Every one of these is in Dr Nish’s hands, in the same room. Follow a link for the detail.
Sofwave™ Brow Lift
Firms the skin across brow and temple. Eyebrow named in the FDA clearance. One session, no needles.
720 PDO Brow Threads
Carries a dropped brow tail back up, there and then. Holds 12–18 months.
Anti-Wrinkle Brow Lift
Eases the muscles dragging the brow down. 10 minutes; 3–4 months.
Staged Plan
Threads carry the brow up; Sofwave™ firms the skin at a later visit. For a brow that has done both.
Sofwave™ Face, Neck & Brow
When the brow is one part of a face that has slackened all over.
Upper Blepharoplasty Opinion
When the hood is stretched lid skin. A referral, not a thread sold into the wrong problem.
Suitability
Am I A Good Candidate
For A Non-Surgical Brow Lift?
You, if the mirror shows a heavier lid than you feel, eye make-up that no longer shows, or a brow tail that has slipped. Which answer fits is settled at the assessment, not before.
Good Candidates
- A brow tail that has slipped towards the outer corner of the eye
- A hood over the outer lid that disappears when you lift the brow with a finger
- Being asked if you are tired when you are not
- Eyeliner or shadow that vanishes into the fold by mid-morning
- Wanting a more open eye without a cut, a scar or a general anaesthetic
- Any skin tone; assessed on the brow, not on age
- Holding a past surgical brow lift where the skin has begun to slacken again
May Not Be Suitable
- A hood that stays put when the brow is lifted: lid skin, where non-surgical treatment may not achieve what surgery can
- Very fine temple skin with little beneath it, where a thread could be felt or seen
- A skin infection, stye or active eczema at the brow or temple
- Pregnancy, or while breastfeeding
- Anticoagulant medication that has not been reviewed first (threads)
- A pacemaker, or metal plates or implants near the brow (Sofwave™)
2 minutes, a mirror and one finger settle most of it. Dr Nish does that test at your assessment, £50 and redeemable in full against treatment, and names which brow you have before any treatment is mentioned. If it is the eyelid, you hear that too.
Step by step
Your Brow Assessment
Every step below is Dr Nish, from the first look to the review.
The Finger Test
Brow at rest, brow raised, brow lifted by Dr Nish’s finger. You watch the hood in the mirror and see for yourself whether it is brow or lid. £50, redeemable in full against treatment.
The Verdict
Slackened, dropped, muscle-held, or lid skin, in plain words. Then the matching plan: Sofwave™, 720 brow threads, a small anti-wrinkle dose, a staged plan, or a referral for an upper blepharoplasty opinion.
1. Sofwave™ Brow
Numbing cream on, then about 20 minutes of ultrasound passes along each brow and across the temples. Slightly pink; make-up on before you leave.
2. PDO Brow Threads
Local anaesthetic at the temple. Each 720 thread enters just inside the hairline and is set along a planned line to the brow tail. You hold the mirror as the height is set. About 30 minutes.
+ Anti-Wrinkle Brow Lift
Where muscle is part of it: a few tiny injections to the muscles pulling the brow down. 10 minutes. The brow rises over the following fortnight.
Reviews
Threads at 4 weeks, once the swelling has gone. Sofwave™ at 12 weeks, when the collagen shows. Dr Nish compares the brow with the assessment and sets the next date.

Non-Surgical Brow Lift Aftercare
— What to Expect
After Sofwave™ the brow may be pink for an hour or two; keep away from heat and hard exercise for 48 hours and that is it. After brow threads expect a puffy, tender temple for about 2 days, then 2 weeks of protecting the brow from pressure. After anti-wrinkle, nothing. You leave with written instructions and Dr Nish’s own number.

- Keep eye make-up and brow pencil off the entry points for 24 hours
- SPF 50 over the brow and temple every morning
- Paracetamol if the temple aches
- Stay upright for 4 hours after an anti-wrinkle brow lift

- Tight glasses, sunglasses pushed up onto the head, headbands and hats for 2 weeks after threads
- Alcohol for the first 48 hours after threads

Clinical Evidence
What the Research Shows
The eyebrow is one of the areas Sofwave™ is FDA-cleared to lift by name, and PDO thread lifting has a peer-reviewed record of its own.
US FDA 510(k) K211483, November 2021
The FDA cleared Sofwave™ to improve facial lines and wrinkles, lift the eyebrow, and lift lax under-chin and neck tissue in adults aged 22 and over. The supporting study ran at 5 US sites with 80 treated patients and 467 treated areas: at 3 months, 80% of eyebrow patients and 85% of under-chin and neck patients were rated improved, with no serious adverse events. FDA 510(k) summary K211483, decision 15 November 2021
Journal of Cosmetic and Laser Therapy, 2018
Patients followed for 24 months after barbed PDO thread lifting showed lifting of 3–10 mm and high satisfaction, with a complication rate of about 4.8%. Ali, J Cosmet Laser Ther 2018;20(1):41–49
Dermatologic Surgery, 2021
In a prospective multicentre study of a single Sofwave™ treatment in 60 patients, 88% showed improvement on the investigator’s global assessment at 12 weeks and 86% improved on the Fitzpatrick wrinkle scale, with no device-related adverse events. Why Dr Nish Shah
Why Medisha for a Brow Lift
— Six Reasons, One Doctor
The brow is a few centimetres of skin over bone, right next to the eye. There is no margin for a thread placed on the wrong line, and no point treating a hood that is really the eyelid. Every argument for doing it here comes down to the same person.
Lines Planned Around the Temple
A brow thread lifts along one line. Dr Nish marks that line against your temple, hairline and the tail you want raised before any anaesthetic goes in.
He Teaches This
Senior International Clinical Trainer at Derma Medical. Derma Medical is one of the UK’s largest aesthetics training academies, and other doctors learn brow threads from him there. Key Opinion Leader and Brand Ambassador for PDO threads.
Sofwave™ Brow Is One Visit
About 20 minutes under numbing cream, no needles and nothing to hide afterwards. Medisha is an official certified Sofwave™ partner and the handpiece stays in Dr Nish’s hand, not delegated.
Threads and Ultrasound in the Same Room
A clinic with only a device tightens every brow. A clinic with only threads lifts every brow. Here both sit on the same trolley, so the brow picks the tool.
Told When It Is the Eyelid
Stretched upper-lid skin needs a surgeon. If that is you, you hear it at the assessment and leave with a referral, not a treatment that cannot open the eye.

What causes a heavy brow and hooded eyes?
Hooded eyes can be opened without surgery when the hood comes from the brow, but not when it is stretched eyelid skin. A brow that has settled because the skin over it has slackened is tightened with Sofwave™, whose FDA clearance names the eyebrow, in one session. A brow tail that has slipped onto the lid is carried back up with 720 PDO brow threads, and the change is there in the mirror before you go home. A brow that muscle is dragging down is eased with a small anti-wrinkle dose. If the fold stays when the brow is lifted by hand, it is lid skin. Dr Nish will say so and point you towards an upper blepharoplasty opinion rather than treat around it.
Yes, Sofwave™ holds FDA clearance 510(k) K211483, granted in November 2021, and lifting the eyebrow is one of the indications named in it. The clearance rested on a five-site US study of 80 patients, with no serious adverse events reported. The device delivers parallel-beam ultrasound to about 1.5 mm, the depth where collagen is made, with SofCool™ cooling protecting the surface. A brow session takes around 20 minutes, with no needles and nothing to recover from. Change shows from about 12 weeks and goes on improving to 6 months. At Medisha, an official certified Sofwave™ partner clinic, Dr Nish Shah MD maps and treats the brow himself.
Sofwave™ suits a brow that has slackened and brow threads suit a brow that has dropped, and the finger test at your assessment tells you which. Lift the outer brow with a fingertip. If it only needs a few millimetres and the skin over it is the problem, tightening is usually enough, with no downtime. If the brow tail is resting on the bone or below it, no amount of tightening moves it; a thread does. A brow that has both dropped and slackened gets a staged plan: threads first, Sofwave™ at a later visit, in the order Dr Nish sets. Results are individual and may vary.
A Sofwave™ brow lift at Medisha Clinic is £700, and a 720 PDO brow thread lift is £750. Where the brow needs both, the staged plan is sequenced by Dr Nish and quoted after consultation. An anti-wrinkle brow lift is priced on the anti-wrinkle page. If the brow is one part of a whole face that has slackened, Sofwave™ face, neck and brow is £2,200. Both reviews are part of the price. The £50 consultation with Dr Nish is redeemable in full against treatment. Medisha is at 58 South Molton Street in Mayfair (W1K 5SL), 3 minutes on foot from Bond Street station.
An anti-wrinkle brow lift raises the brow by about 1 to 2 mm, and only when muscle is part of the problem. The frown muscles and the muscle ringing the eye pull the brow down; the forehead pulls it up. A small dose placed in the downward-pulling muscles tips that balance, and the brow rises over about 2 weeks. The effect lasts 3–4 months. It does not tighten slack skin and cannot move a brow tail that has dropped onto the lid. At Medisha it is used alone for a mildly heavy outer brow, and added to Sofwave™ or threads where the brow needs more.
Brow threads hold for 12 to 18 months, a Sofwave™ brow is maintained yearly, and an anti-wrinkle brow lift fades at 3–4 months. The timelines differ, so here is each. With threads the brow tail is up on the day and the temple is puffy for about 48 hours. By 4 weeks the lift has settled and Dr Nish reviews it. The threads have dissolved by 6 to 9 months, and the collagen left behind carries the position to 12 to 18 months. With Sofwave™ the first few weeks show almost nothing. The outer lid starts to open from about 12 weeks, is still improving at 6 months, and is kept there with one session a year. Both review dates and the first top-up are written into your plan.
Brow threads are placed under local anaesthetic and a Sofwave™ brow is done under numbing cream, so discomfort is low and downtime is 2 days at most. Sofwave™ feels like pulses of deep warmth across the brow and temple for about 20 minutes. You can put make-up on and go back to work the same afternoon. Brow threads enter through a tiny point just inside the hairline on each side. Expect a tender, puffy temple for around 48 hours, sometimes a bruise, and 2 weeks of keeping pressure off the brow. Most people are at their desk again the following morning. An anti-wrinkle brow lift is a few tiny injections with nothing to recover from. Written aftercare and Dr Nish’s direct number go home with you.
No — a non-surgical brow lift puts the brow back where it used to sit, it does not raise it above that. Sofwave™ tightens the skin without touching the muscles, so expression is unchanged. Brow threads are adjusted live, with you watching in the mirror, so the lift is set at a natural height. An anti-wrinkle brow lift uses a small dose placed to release the muscles that pull the brow down, not the ones that lift it, so the forehead still moves. Dr Nish shows you the planned height before treating.
Lift your brow gently with a finger in the mirror. If the hooding over the outer eye disappears, the problem is brow position and a non-surgical brow lift will help. If the fold of skin stays on the lid, it is excess eyelid skin and only eyelid surgery removes it. Many people have some of each, and the brow part can still be treated non-surgically. Dr Nish does this test at consultation and tells you plainly which you are and what each answer will achieve.
Dr Nish says it plainly at the assessment, and you leave with a referral for an upper blepharoplasty opinion. Stretched upper-eyelid skin is removed by a surgeon; no ultrasound, thread or injection takes skin away, and non-surgical treatment may not achieve what surgery can. No brow treatment is sold here into an eyelid problem. Many people have a little of both. Where part of your hood is a dropped brow, that part can still be treated here, and the plan says which part is which.
Because the brow is assessed, treated and reviewed by one doctor who performs both non-surgical answers and teaches one of them. Dr Nish Shah MD has placed thousands of PDO thread lifts. At Derma Medical he is the Senior International Clinical Trainer, and other doctors learn thread lifting from him there. He is also Key Opinion Leader and Brand Ambassador for PDO threads. Medisha is an official certified Sofwave™ partner, so a brow that needs tightening rather than lifting is treated in the same room. His count of over 30,000 non-surgical treatments places him as one of the most experienced aesthetic doctors in the UK and London. When the hood is eyelid skin, he says so and refers you. The clinic is rated 5.0 on Google from 128 reviews and sits at 58 South Molton Street in Mayfair, with a second base in Marylebone.
