Profhilo is an injectable bio-remodelling treatment: 64mg of ultra-pure, NAHYCO-stabilised hyaluronic acid placed at five Bio Aesthetic Points on each side of the face, where it spreads through the dermis and triggers your skin to produce new collagen, elastin and even fat-cell stem cells. It is not a filler and not a standard skin booster — it tightens and hydrates without adding volume. At Medisha Clinic it is performed only by Dr Nish Shah MD, a medically trained doctor who trains other doctors in skin-regeneration injectables.

The Treatment
What Is Profhilo?
Profhilo is the first injectable made from NAHYCO hyaluronic acid — high and low molecular-weight HA thermally bonded together without the chemical cross-linkers used in fillers and most skin boosters. Each treatment delivers 64mg of this ultra-pure HA, one of the highest concentrations of any injectable, through just five Bio Aesthetic Points on each side of the face because it is not cross-linked, it does not sit where it is placed. It spreads through the dermis over the following days.
As it spreads it does two things. It hydrates intensely — the low-weight fraction binds many times its weight in water — and it triggers bio-remodelling: fibroblasts and fat-cell (fat) stem cells respond by producing new collagen types I, III, IV and XVII and new elastin. The net effect is skin that is measurably firmer, more elastic and more luminous, with sagging improved along the jawline, cheeks and neck because no volume is added, the face is not reshaped. It simply looks well-rested and well-hydrated in a way no cream can achieve.
The result: visible improvement in hydration and glow from four weeks, peak firmness and tightening eight weeks after the second session, lasting six to nine months, from a 15–20 minute treatment with minimal downtime. At Medisha Clinic every session is performed by Dr Nish Shah MD using The Medisha Method™.
The Technique
The Medisha Method™ — Why BAP Placement Is Not a Template
The Medisha Method is Dr Nish Shah’s four-step protocol, applied to profhilo as: sagging mapping, individualised BAP placement, slow deposit delivery at the correct depth, and second-session calibration. Profhilo’s published protocol gives five standard points per side. The manufacturer intends them as a starting map, not a rule. The best results come from adjusting them to the individual face. That adjustment is what separates a doctor who understands the anatomy from an injector following a diagram.
- Step 1 — Laxity mapping. The face and neck are assessed for where sagging, dehydration and thinning actually sit, and photographed under standardised light. A face with jowl-line sagging and a face with mid-cheek crepey texture should not receive identical placement.
- Step 2 — Individualised BAP placement. The five points per side are shifted, within the anatomically safe zones, towards the areas of greatest need identified in step 1 — and away from vascular danger zones such as the facial artery’s course.
- Step 3 — Slow deposit delivery at the correct depth. Each 0.2ml deposit is placed just under the skin, slowly, after checking the needle is clear of blood vessels. Too superficial and the bump persists for days and the spread is poor. Too deep and the product is wasted below the dermis.
- Step 4 — Second-session calibration. At four weeks the response is reviewed against the baseline photographs and the second session’s points are calibrated to the areas that responded least. This is why Medisha patients rarely need a third session.
- Performed by the trainer. Dr Nish Shah MD teaches skin-regeneration injectables to other doctors as Senior International Clinical Trainer at Derma Medical and is the Official UK Trainer for Sunekos. Every Profhilo session at Medisha is his.
Six Questions to Ask Any Clinic Before Profhilo
Profhilo is one of the most widely offered injectables in London and is frequently delivered by following the standard diagram. Before you book anywhere — including here — ask these six questions.
1. Is the person treating me a specialist in skin-regeneration injectables — or a general injector? At Medisha: Dr Nish Shah MD is Senior International Clinical Trainer, Official UK Trainer for Sunekos, and Brand Ambassador & Key Opinion Leader for dermal fillers.
2. Is that person a medically trained doctor? At Medisha: Yes — Dr Nish Shah MD, every session.
3. Will my BAP points be adjusted to my face, or injected from the standard diagram? At Medisha: Individualised at consultation — step 2 of The Medisha Method.
4. Is my second session calibrated to how I responded to the first? At Medisha: Yes — step 4 of The Medisha Method, with photographs at both visits.
5. Will you tell me if Sunekos, polynucleotides or filler would suit me better? At Medisha: Yes — Dr Nish performs all of them and recommends on your skin.
6. Is the price fixed with the protocol discounted? At Medisha: £350 per session, £700 for the two-session protocol, £50 consultation redeemable.

Skin-quality injectables compared
Profhilo vs Sunekos vs Polynucleotides vs Skin Boosters
Four injectables improve skin quality in London clinics and they are constantly confused. They work by different mechanisms and suit different skin. Dr Nish performs all four, which is why his recommendation is based on your skin rather than his stock.

Treatment areas
Areas Treated with Profhilo at Medisha Clinic
Each area is treated as two sessions four weeks apart at £350 per session. Areas can be combined in one visit.
Face
The standard indication: sagging along the jawline and mid-face, dehydration and dullness. Ten individualised BAP points per session.
Neck
Crepey, lax neck skin and horizontal lines. Profhilo’s spreading action suits the neck particularly well. Often combined with the face.
Décolletage
Sun-damaged, thinning chest skin. Bio-remodelling restores density and luminosity where fillers are unsuitable.
Hands
Thin, veiny skin on the backs of the hands. Profhilo restores plumpness and hydration without the lumpiness filler can create.
Jawline & Lower Face
Early jowl-line softening and loss of definition from skin sagging rather than volume loss. Where sagging is more advanced, PDO threads may be combined.
Combination with Sunekos
Profhilo for hydration and tightening, followed by Sunekos for amino-acid regeneration of texture and the under-eyes. Priced individually.
Suitability
Am I A Good Candidate For Profhilo?
Profhilo suits patients from their 30s onwards whose skin has become lax, dull or dehydrated — the ‘tired’ look — without significant volume loss. Dr Nish will assess whether Profhilo, Sunekos, filler or a combination addresses what you are actually seeing.
Good Candidates
- Early to moderate skin sagging on the face, neck or jawline
- Dull, dehydrated or ‘tired-looking’ skin
- Loss of firmness and elasticity with age or sun exposure
- Wanting a refreshed look without changing facial shape
- Patients who want no visible sign of ‘treatment’
- Maintenance between or after other treatments
May Not Be Suitable
- Significant volume loss — dermal filler is needed
- Thin, crepey under-eye skin — Sunekos Performa is more appropriate
- Advanced sagging needing a lift — PDO Thread Lift first
- Pregnancy or breastfeeding
- Active infection or inflammatory skin disease in the area
The best way to know is a consultation with Dr Nish. He will assess your anatomy and goals in person and give you an honest recommendation. The consultation is £50, redeemable in full against treatment, with no obligation to proceed; if you are suitable and would like to go ahead, treatment can be performed the same day.
Step by step
The Profhilo Process
Every profhilo at Medisha Clinic follows The Medisha Method™ — the protocol Dr Nish teaches to other doctors.
Consultation & Laxity Mapping
Dr Nish assesses where sagging, dehydration and thinning sit, photographs under standardised light and confirms Profhilo is the right product. £50, redeemable against treatment.
Preparation
Skin cleansed with antiseptic and the individualised BAP points marked. Avoid alcohol, aspirin, ibuprofen, fish oil and vitamin E for 48 hours beforehand.
Anaesthetic (Rarely Needed)
Ten small injections per session. Most patients need nothing. Topical cream is available on request
Session One
Each 0.2ml deposit placed slowly, after checking the needle is clear of blood vessels, at the correct just under the skin depth at the mapped points. 15–20 minutes. Small bumps are visible for a few hours.
Session Two — Four Weeks Later
Response reviewed against baseline photographs and the points calibrated to the areas that responded least. This is the session that produces the tightening.
06
Review
Approximately four weeks after session two, when bio-remodelling peaks, Dr Nish reviews the result and advises on maintenance — usually one session every six months.

- Attend session two at exactly four weeks
- Attend your review
Avoid for 24–48 hours
- Make-up on the treated area for 12 hours

- Alcohol
- Facials or active skincare on the area for one week
Clinical Evidence
What the Research Shows
Profhilo has one of the strongest evidence bases of any skin-quality injectable, with instrumental and skin-sample analysis studies rather than satisfaction surveys alone.
Journal of Cosmetic Dermatology
Two-session Profhilo protocols produced statistically significant improvements in skin elasticity, firmness and hydration on instrumental measurement at eight weeks, with skin-sample analysis confirming new collagen and elastin.
IBSA Clinical Data
NAHYCO thermal stabilisation produces an HA complex free of chemical cross-linkers, demonstrating superior biocompatibility and tissue spread compared with cross-linked HA products.
British Journal of Dermatology
In a multi-centre study over 85% of patients reported improved skin quality, firmness and hydration at two months, with the majority maintaining improvement at six months.

Profhilo is an injectable bio-remodelling treatment made from NAHYCO hyaluronic acid — high and low molecular-weight HA thermally bonded together with no chemical cross-linkers. Each treatment delivers 64mg of this ultra-pure HA through five Bio Aesthetic Points on each side of the face (ten injections in total) because the HA is not cross-linked it does not stay where it is placed: over the following days it spreads through the dermis, hydrating intensely and prompting fibroblasts and fat-cell stem cells to produce new collagen types I, III, IV and XVII and new elastin. That response is what the manufacturer calls bio-remodelling, and it is what makes Profhilo different from a filler (which adds volume) or a standard skin booster (which only hydrates). Two sessions four weeks apart are needed. Results appear from four weeks and peak eight weeks after the second session, lasting six to nine months. At Medisha Clinic every session is performed by Dr Nish Shah MD using The Medisha Method.
At Medisha Clinic in Mayfair a single Profhilo session is £350 and the recommended two-session protocol is £700. Neck and hands are £350 per area per session. Maintenance after a completed protocol is £350. Consultation with Dr Nish is £50, redeemable in full against treatment, so if you go ahead it costs nothing. A review is included after the protocol. The clinic is at 58 South Molton Street, Mayfair, London W1K 5SL, three minutes from Bond Street station.
Two, four weeks apart, and the four-week gap matters. The first session starts the bio-remodelling response. The second, delivered when that response is at its peak, amplifies it substantially. Studies and clinical experience agree that one session produces a noticeable but short-lived improvement in hydration, while two produce the tightening and firming Profhilo is known for. A third session is occasionally useful for very lax or sun-damaged skin. After the protocol, one maintenance session every six months keeps the result. At Medisha, because the second session is calibrated to your response to the first, patients rarely need more than that.
Most patients notice better hydration and a healthier glow around four weeks after the first session — just as the second session is due. After the second session the tightening builds, reaching its peak roughly eight weeks later, about twelve weeks from the start. That is when firmness, elasticity and luminosity are at their best and when your review photographs are taken. Results typically last six to nine months, varying with age, sun exposure and lifestyle because Profhilo remodels rather than fills, the decline is gradual. Your skin does not suddenly return to baseline, and a single maintenance session every six months holds you close to peak.
No. Profhilo and dermal fillers are both hyaluronic acid, but that is where the similarity ends. Fillers are cross-linked so they hold their shape and stay where they are placed. That is how they add volume to a cheek or reshape a nose. Profhilo is deliberately not cross-linked so that it spreads through the dermis instead of sitting in one spot. It adds no volume and cannot reshape anything. What it does is improve the quality of the skin itself — firmness, elasticity, hydration, luminosity, and tighten mild to moderate sagging. If you have lost volume you need filler. If your skin has become lax and dull you need Profhilo. Many patients need both, and they can be combined in a planned programme. Dr Nish performs both and will tell you which addresses what you are seeing.
Both are injectable skin-quality treatments performed by Dr Nish, but they work differently and suit different skin. Profhilo is pure NAHYCO HA that spreads through the dermis, hydrating intensely and triggering bio-remodelling. It excels at sagging and dehydration on the face and neck and needs two sessions. Sunekos combines HA with six amino acids that fibroblasts use directly to build collagen and elastin. It excels at crepey texture, fine lines and thin under-eye skin, is safe under the eyes where Profhilo is not used, and needs four sessions. In practice: lax and tired-looking skin — Profhilo. Crepey, fine-lined or thin skin, or under-eye concerns — Sunekos. Both, in sequence, for comprehensive skin quality. Dr Nish is the Official UK Trainer for Sunekos and performs both, so his recommendation is based on your skin.
For skin tightening and regeneration, yes, and the mechanism explains why. Traditional skin boosters use lightly cross-linked HA injected as many small deposits across the face. The HA stays where it is placed and hydrates locally, giving a temporary plumped, dewy look that fades in four to six months. Profhilo uses non-cross-linked NAHYCO HA at ten points, which spreads through the tissue and triggers a genuine collagen and elastin response — bio-remodelling. That tightens the skin and lasts six to nine months. Skin boosters remain a reasonable choice for a short-term glow before an event. For lasting improvement in firmness and quality, Profhilo is the stronger treatment and the better-evidenced one.
Profhilo is one of the most comfortable injectables: only ten injections per session, each a brief sting lasting a second or two, and most patients need no anaesthetic at all — topical cream is available if you prefer. Afterwards you will have a small raised bump at each point, which is normal and settles within 4–24 hours. Mild redness clears within a day and occasional bruising within a week. Most patients go straight back to work. For 24–48 hours avoid make-up on the area (12 hours), massaging the points, strenuous exercise, saunas, sunbeds, alcohol and active skincare. There is no restriction on flying or normal activities.
Yes. The neck is arguably where Profhilo performs best of all. Neck skin is thin, becomes crepey and lax early, and has few good treatment options; Profhilo’s spreading action suits it perfectly and the result is visibly firmer, smoother neck skin. The backs of the hands respond similarly, regaining plumpness and hydration without the lumpiness that filler can produce there. The décolletage benefits from restored density and luminosity in sun-damaged skin. Each area is treated as two sessions four weeks apart at £350 per session, and areas are often combined in one visit — face and neck together being the most common.
Yes, and the best skin results at Medisha usually come from a planned combination. Profhilo followed by Sunekos pairs hydration and tightening with targeted amino-acid regeneration — Dr Nish’s most-requested skin-quality programme. Profhilo alongside dermal filler treats skin quality and volume loss at the same time. Profhilo after a PDO Thread Lift improves the skin over the repositioned tissue so the lift looks better and lasts longer. Profhilo with anti-wrinkle injections addresses skin texture and expression lines together. Different injectables are usually spaced two to four weeks apart so each can be assessed; Dr Nish will design the sequence at consultation.
Profhilo has an excellent safety record and, because it contains only ultra-pure HA with no cross-linkers, an unusually low rate of reactions. Expected, temporary effects are the small bumps at each point for 4–24 hours, mild redness for a day and occasional bruising for up to a week. Uncommon effects include bumps persisting for a few days (usually from placement that is too superficial — avoided by correct depth), transient swelling, and, rarely, infection at an injection point. Allergic reaction is very rare because Profhilo is placed just under the skin at defined points away from major vessels, and is not a volumising product, the vascular complications associated with dermal fillers are far less relevant. But the injector still needs to know where the facial artery runs, which is why every Medisha session is performed by a medically trained doctor.
Because the person injecting is the person who trains other doctors. Dr Nish Shah MD is a medically trained doctor, Senior International Clinical Trainer at Derma Medical, Official UK Trainer for Sunekos, and Brand Ambassador and Key Opinion Leader for dermal fillers with a global manufacturer. He performs every Profhilo session himself using The Medisha Method — mapping your sagging, individualising the BAP points to your face rather than injecting the standard diagram, and calibrating the second session to how you responded to the first. He also performs Sunekos, polynucleotides and dermal filler, so his recommendation is based on your skin rather than on what he sells. Two-session protocol £700; £50 consultation redeemable. 58 South Molton Street, Mayfair, London W1K 5SL, three minutes from Bond Street. Also Marylebone.
