Tear Trough Filler London
Tear trough filler removes the hollow beneath the eye — and the shadow it casts, which is the commonest real cause of dark circles — with a small amount of hyaluronic acid placed on bone. The under-eye is the one place where the choice of filler matters as much as the injector: most fillers absorb water and swell. Under the thinnest skin on the body that means puffiness and a blue tint. At Medisha Clinic Dr Nish uses Teosyal PureSense Redensity 2, the first filler designed specifically for the tear trough, which barely absorbs water and does not go blue — placed on bone by cannula using The Medisha Method™ under-eye protocol. Results are immediate and last 12–18 months. Performed only by Dr Nish Shah MD, a medically trained doctor, specialist in under-eye and advanced facial filler, Brand Ambassador & Key Opinion Leader for dermal fillers, and the international trainer who teaches this procedure to other doctors.

The Treatment
What Is A Tear Trough Filler?
Tear trough filler restores volume to the groove that runs from the inner corner of the eye down onto the cheek — the tear trough groove, or tear trough. The groove exists because a ligament tethers the skin there to the bone. As the fat pads beneath the eye deflate and descend with age, or from genetics in the young, the tether becomes a visible hollow. That hollow casts a shadow, and that shadow is what most people mean by dark circles.
A small amount of hyaluronic acid — typically half a millilitre to a millilitre across both sides — placed deep on the bone beneath the muscle lifts the floor of the hollow, removes the shadow and restores a smooth line from lid to cheek. Nothing is done to the skin. The darkness goes because the shadow goes. Two things make the under-eye different from every other filler area. The skin is the thinnest on the body, so anything that swells or sits too high shows, and it drains slowly, so a water-absorbing filler stays puffy for weeks. This is why the product matters here as much as the injector — and why Dr Nish uses Teosyal PureSense Redensity 2, the first filler designed specifically for this area, and nothing else under the eye.
The result: a rested, unhollowed under-eye the moment the procedure ends, settling at two weeks, lasting 12–18 months, fully reversible with hyaluronidase. Filler is not the answer to pigmented dark circles and only part of the answer to vascular ones. This is why the diagnosis at consultation is the most important part of the treatment. At Medisha it is doctor-only, performed exclusively by Dr Nish Shah MD using The Medisha Method™ under-eye protocol. He is a medically trained doctor, specialist in under-eye and advanced facial filler, Brand Ambassador & Key Opinion Leader for dermal fillers, and the international trainer who teaches this procedure to other doctors.
The Technique
The Medisha Method™ — The Under-Eye Protocol
The Medisha Method is Dr Nish Shah’s four-step protocol, applied to tear trough filler as: cause diagnosis with midface assessment, Redensity 2 product selection, deep on-bone cannula placement checking each deposit is clear of blood vessels, and conservative live recontouring. More filler is dissolved from under eyes than from anywhere else in London, and the causes are always the same: wrong diagnosis, wrong filler, wrong plane, too much. The under-eye protocol removes each in turn — and the filler is a step in the protocol, not an afterthought.
- Step 1 — Cause diagnosis with midface assessment. Dark circles are a hollow, a vascular show-through, a pigment, or a mix. Stretch and light tests at consultation establish which. The cheek is assessed at the same time, because a deflated midface deepens the trough — in some patients cheek filler alone corrects it without touching the under-eye.
- Step 2 — Teosyal PureSense Redensity 2. The under-eye needs a filler that will not swell and will not show blue. Teosyal PureSense Redensity 2 was designed for exactly this: a low HA concentration in a mix of cross-linked and non-cross-linked gel, so it binds very little water. Low stiffness so it spreads smooth over thin skin. Lidocaine for comfort, and a complex of amino acids, antioxidants, minerals and vitamin B6 that supports the skin itself. It is the filler Dr Nish uses for every tear trough — see the comparison below.
- Step 3 — Deep on-bone placement by cannula, checking each deposit is clear of blood vessels. Filler goes beneath the muscle under the eye directly on the bone of the eye socket — never into the skin. A blunt-tipped cannula through a single entry point, checking each deposit is clear of blood vessels and slow tiny deposits, keeps the blood vessels under the eye intact and cuts bruising by around 70% compared with needle.
- Step 4 — Conservative live recontouring. The area is reassessed in daylight and from below after every deposit. The end point is the disappearance of the shadow, not a full trough. Anything further is added at the 4-week review rather than over-corrected on the day.
- Hyaluronidase in the room, and only a doctor. The enzyme that dissolves HA is prepared before treatment begins. Dr Nish teaches this exact protocol to other doctors as Senior International Clinical Trainer at Derma Medical.
Six Questions to Ask Any Clinic Before Under-Eye Filler
Under-eye filler is offered by hundreds of London clinics, and a great deal of it ends up being dissolved. Before you book anywhere — including here — ask these six questions.
1. Is the person performing my procedure an absolute specialist in under-eye filler — not a general injector? At Medisha: Dr Nish Shah MD is a medically trained doctor, specialist in under-eye and advanced facial filler, Brand Ambassador & Key Opinion Leader for dermal fillers, and the international trainer who teaches this procedure to other doctors, and creator of The Medisha Method.
2. Is that person a medically trained doctor? At Medisha: Yes — Dr Nish Shah MD, every time.
3. Which filler will you use under my eye, and was it designed for the under-eye? At Medisha: Teosyal PureSense Redensity 2 — the first filler designed specifically for the tear trough. Low-swelling and resists the blue tint.
4. Will you diagnose whether my dark circles are a hollow, vascular or pigment — and check my cheeks — before injecting? At Medisha: Yes — step 1 of The Medisha Method. If filler is not the answer, Dr Nish will say so.
5. Is it placed on bone by cannula checking each deposit is clear of blood vessels, and is hyaluronidase in the room? At Medisha: Yes — step 3; hyaluronidase prepared at every appointment.
6. Is the price fixed with a review included? At Medisha: £450 with 4-week review and refinement included; £50 consultation redeemable.

Redensity 2 Filler vs Polynucleotides vs Sunekos Performa vs Eyelid Surgery
Under-eye concerns bring patients to four different treatments, and clinics tend to recommend whichever they offer. Dr Nish performs the first three and refers for the fourth, so the recommendation follows the cause of your dark circles rather than the menu.

What it corrects
What Tear Trough Filler Can Correct at Medisha Clinic
One fixed price of £450 covers the tear trough on both sides with Teosyal PureSense Redensity 2, including the 4-week review. Cheek filler can be added where the midface is contributing.
Under-Eye Hollows
The primary indication: a sunken groove beneath the eye that makes you look tired however well you sleep. Volume restored on bone removes the hollow.
Shadow Dark Circles
Darkness caused by the shadow of the hollow rather than by the skin. Remove the hollow and the darkness goes — often dramatically.
Tired or Aged Appearance
The combination of hollow and shadow is the single biggest driver of a chronically tired look, addressed at its structural cause.
Lid–Cheek Step
A visible line or step where the lower lid meets the cheek, smoothed into a single youthful curve — Redensity 2’s low stiffness suits this junction.
Midface Deflation
Where a flattened cheek is deepening the trough, cheek filler (from £350) lifts the floor of the hollow and may correct it without any under-eye injection.
Correction of Previous Filler
Assessment and, where appropriate, dissolving and re-treatment with Redensity 2 of puffy, lumpy or blue under-eye filler placed elsewhere.
Suitability
Am I A Good Candidate For Tear Trough Filler?
Tear trough filler suits patients with a genuine hollow beneath the eye and reasonable under-eye skin, without significant puffiness or fat bags. Dr Nish will diagnose the cause of your dark circles and assess your midface before recommending it.
Good Candidates
- A visible hollow or groove beneath the eye
- Dark circles that are a shadow — they lighten when the area is lit from below
- A tired look that no amount of sleep improves
- A visible step between lower lid and cheek
- Reasonably thick under-eye skin
- No fat bags or fluid puffiness
- Previous under-eye filler elsewhere that swelled or went blue
May Not Be Suitable
- Puffiness, fat bags or fluid retention — filler makes these worse. Eyelid surgery may be the answer
- Dark circles that are purely pigment
- Very thin, crepey skin without a hollow — Sunekos Performa or polynucleotides first
- Previous lower-eyelid surgery — assessed case by case
- Pregnancy or breastfeeding
The best way to know is a consultation with Dr Nish. He will assess your anatomy and goals in person and give you an honest recommendation. The consultation is £50, redeemable in full against treatment, with no obligation to proceed; if you are suitable and would like to go ahead, treatment can be performed the same day.
Step by step
The Tear Trough Filler Process
Every tear trough filler at Medisha Clinic follows The Medisha Method™ — the protocol Dr Nish teaches to other doctors.
Consultation & Diagnosis
Dr Nish establishes whether your dark circles are a hollow, vascular, pigment or a mix, assesses the cheek and skin thickness, and photographs from front and below under standardised light. £50, redeemable against treatment.
Preparation
Skin cleansed with antiseptic and the cannula entry point marked. Avoid alcohol, aspirin, ibuprofen, fish oil and vitamin E for 5–7 days beforehand. Hyaluronidase is prepared.
Anaesthetic
Topical anaesthetic for 20 minutes and a small bleb of local anaesthetic at the entry point. Redensity 2 contains lidocaine.
Placement — Teosyal PureSense Redensity 2
A blunt-tipped cannula through a single entry point per side delivers small, checked deposits of Redensity 2 deep on the bone of the eye socket beneath the muscle. 10–15 minutes.
Live Assessment
Reassessed in daylight and from below after each deposit. The end point is the disappearance of the shadow, not a ‘full’ look. Aftercare and Dr Nish’s direct number provided.
Complimentary 4-Week Review
Swelling has settled and the true result is visible. Any refinement is added now at no extra charge — the safe direction under the eye is always to add later, never to over-fill on the day.

— What to Expect
Because Redensity 2 binds very little water, swelling is usually mild and settles within 2–3 days rather than the weeks common with other fillers under the eye. Bruising, if any, settles within 5–10 days and can be covered with make-up after 24 hours. The result is considered final at two weeks. Contact Dr Nish the same day if you notice skin blanching or mottling, disproportionate pain, or any change in vision.
Do
- Sleep on your back with your head raised for the first two nights
- Apply a cool (not frozen) compress gently for the first 24 hours
- Take paracetamol if needed (avoid ibuprofen and aspirin for 48 hours)
- Wear sunglasses outdoors and SPF once healed
- Attend your complimentary 4-week review
- Keep Dr Nish’s number for the first 48 hours
Avoid for 2 weeks
- Rubbing or pressing the under-eye area
- Sleeping face-down or on your side against the eye
- Facials, massage, eye masks or pressure on the area
- Saunas, steam rooms, hot yoga and sunbeds
- Strenuous exercise and alcohol (first 48 hours)
- Eye make-up for 24 hours
Clinical Evidence
What the Research Shows
Tear trough filler is well evidenced, and the literature is unusually consistent on two points: the product must be designed for the area, and the outcome depends on who is injecting.
Aesthetic Plastic Surgery, 2025
A prospective real-world study of Teosyal PureSense Redensity 2 confirmed a ten-year record of efficacy and safety in the tear trough, attributing its suitability to low stiffness and low water absorption that minimise the swelling undesirable in this area.
Journal of Cosmetic Dermatology
Cannula delivery to the tear trough reduced bruising by around 70% compared with needle technique while achieving equivalent correction, supporting cannula as the standard for this area.
Dermatologic Surgery — Safety Review
The under-eye region is the most frequently reported site of filler complications including blue tint under the skin, long-lasting swelling and vascular events, underscoring the need for area-specific products, physician-level anatomy training and on-site hyaluronidase.

Tear Trough Filler FAQ — Answered by Dr Nish Shah
Medically written and reviewed by Dr Nish Shah MD — Medical Director, Medisha Clinic · Senior International Clinical Trainer, Derma Medical · Brand Ambassador & KOL for Dermal Fillers · Official UK Trainer, Sunekos · Last reviewed 19 September 2026
Tear trough filler restores lost volume to the tear trough groove. The hollow that runs from the inner corner of the eye down onto the cheek — using a small, precisely placed amount of reversible hyaluronic acid filler. The groove exists because a ligament tethers the skin to the bone there. As the fat beneath the eye deflates and descends with age, or from genetics in younger patients, that tether becomes a visible hollow that casts a shadow. That shadow is the commonest real cause of dark circles. Filler placed deep on the bone of the eye socket, beneath the muscle, lifts the floor of the hollow so the shadow disappears and the lid flows smoothly into the cheek. Typically 0.5–1ml is used across both sides. Results are immediate, settle at two weeks, last 12–18 months and can be fully dissolved with hyaluronidase. At Medisha Clinic it is a doctor-only procedure performed exclusively by Dr Nish Shah MD with Teosyal PureSense Redensity 2 — the filler designed specifically for the under-eye — using The Medisha Method under-eye protocol.
Dr Nish uses Teosyal PureSense Redensity 2 for every tear trough, and nothing else under the eye. It was the first filler designed specifically for this area and it is built around the two things that go wrong there. First, swelling: most fillers are water-absorbing, and under the thinnest, slowest-draining skin on the body that means puffiness for weeks; Redensity 2 combines a low concentration of cross-linked and non-cross-linked HA so it binds very little water and settles within days. Second, the blue tint under the skin: Redensity 2’s low stiffness lets it lie smooth and it is formulated to resist showing blue through thin skin. It also contains lidocaine for comfort and a complex of amino acids, antioxidants, minerals and vitamin B6 that supports the skin. It has a ten-year clinical track record and a 2025 prospective study in Aesthetic Plastic Surgery describes it as the product of choice for the tear trough. It is CE-marked and used across Europe, Canada and Asia. It is not sold in the United States. Using a lip or cheek filler under the eye is one of the commonest reasons under-eye filler has to be dissolved.
At Medisha Clinic in Mayfair, tear trough filler is £450 — a single fixed price covering both sides, treatment with Teosyal Redensity 2, the immediate assessment and a complimentary 4-week review at which any refinement is included. Consultation with Dr Nish is £50, fully redeemable against treatment, so if you go ahead your total is £450 and there are no other charges. Where a deflated cheek is deepening the trough, cheek filler can be added from £350 (tear trough and cheek together from £800). Correction of under-eye filler placed elsewhere, with hyaluronidase and re-treatment, is quoted at consultation. The clinic is at 58 South Molton Street, Mayfair, London W1K 5SL, three minutes from Bond Street station.
Because the under-eye is, with the nose, the least forgiving filler area on the face. The skin is the thinnest on the body, so anything placed too superficially shows as a lump or a bluish tint. The blood vessels under the eye run through the area and connect to the blood supply of the eye, so an injection into a blood vessel can cause loss of skin from a blocked blood supply or, rarely, visual loss. The fluid drainage is slow, so the wrong product or too much of it causes puffiness that can last months. Managing those risks requires knowledge of the plane beneath the muscle, cannula technique checking each deposit is clear of blood vessels, the right low-swelling product, the judgement to use very little. Hyaluronidase prepared in the room with the competence to use it immediately. Dr Nish Shah is a medically trained doctor and the Senior International Clinical Trainer at Derma Medical, where he teaches this protocol to other doctors. It is codified as The Medisha Method under-eye protocol.
It fixes the commonest type completely and the others partly or not at all, which is why the diagnosis at consultation is the most important part of the treatment. Dark circles have three causes. A hollow casting a shadow: filler removes the hollow, the shadow goes, and the improvement is often dramatic. Vascular show-through — bluish or purple vessels visible through thin skin: filler helps a little by adding a layer over the vessels. But polynucleotides, which thicken the skin and calm the congestion, are the better tool, often used four weeks after filler. Pigment — brown melanin in the skin: filler does nothing, and pigment treatments are needed. Most patients have a mix. Dr Nish uses stretch and light tests at consultation to establish which you have, and will tell you plainly if filler is not the answer.
Not when it is diagnosed, chosen and placed correctly, and puffiness is precisely what the under-eye protocol is designed to prevent. Under-eye puffiness after filler comes from four errors: treating a patient who already has fat bags or fluid retention. Placing the filler in the skin rather than deep on bone. Using too much, and using a water-absorbing filler that swells. At Medisha, patients with true bags are told so and referred for eyelid surgery. Filler goes beneath the muscle on the bone of the eye socket by cannula. Volumes are conservative (typically 0.5–1ml across both sides), and the filler is Teosyal Redensity 2, designed for this area specifically because it binds very little water. The end point is the disappearance of the shadow, not a full trough, and any refinement is added at the 4-week review rather than risked on the day. If you have been left puffy by filler elsewhere, it can be dissolved and re-treated correctly.
Typically 12–18 months, which is longer than most facial filler because the under-eye barely moves and the HA is broken down slowly. Some patients find a result persisting well beyond 18 months, especially after a second treatment. Longevity varies with your metabolism and lifestyle factors such as intense exercise and sun exposure. When the effect fades a small top-up restores it because Dr Nish uses only reversible HA, the filler can also be dissolved at any point if you wish.
Most patients describe pressure rather than pain. Topical anaesthetic is applied for 20 minutes, a tiny bleb of local anaesthetic numbs the single cannula entry point. Redensity 2 contains lidocaine because a cannula is used there is one entry point per side rather than multiple needle passes. Downtime is short: the result is visible immediately, most patients return to work the next day. Because Redensity 2 binds so little water, swelling is usually mild and settles within 2–3 days rather than the weeks common with other fillers here. Bruising, if it occurs, clears within 5–10 days and can be covered with make-up after 24 hours. The final result is assessed at two weeks. For two weeks avoid rubbing the area, sleeping face-down, facials and eye masks. For 48 hours avoid vigorous exercise, alcohol and heat.
They treat different causes of the same complaint. Tear trough filler restores volume to a hollow, removing the shadow it casts. It is immediate, structural and reversible, and it is the treatment for a visible groove. Polynucleotides regenerate the under-eye skin at a cellular level — thickening thin, translucent skin and calming vascular congestion — without adding any volume. They are the treatment for bluish, vascular dark circles and crepey skin, and they build over a course of three to four sessions. Sunekos Performa does a similar job for skin quality via amino acids. Many patients have both a hollow and thin skin and are treated in sequence: Redensity 2 first, then polynucleotides or Sunekos Performa four weeks later. Dr Nish performs all three, so the recommendation follows the diagnosis rather than the menu.
Usually not, and a good clinic will tell you so. True eye bags are fat pads pushing forward pushing forward beneath the eye. Fluid puffiness is fluid congestion. Filler adds volume to an area that already has too much, so it makes both worse, and filler placed into bags is among the most commonly dissolved in London. For genuine fat bags the correct treatment is lower eyelid surgery, and Dr Nish will refer you. For fluid puffiness, lifestyle and fluid measures come first. There is a middle group — a slight bag with a deep trough beneath it. Where very conservative Redensity 2 placed only in the trough can smooth the transition, and Dr Nish assesses this case by case with you looking in the mirror.
This is the area where the risks of filler are most concentrated, which is exactly why it is doctor-only at Medisha and why the product is chosen for the area. Expected, temporary effects: mild swelling for 2–3 days with Redensity 2, bruising for up to 10 days, mild tenderness. Uncommon risks: a blue tint under the skin (the blue tint under the skin). Prolonged puffiness (from a water-absorbing product, too much of it or fluid disruption — avoided by Redensity 2, conservative volume and cannula). Asymmetry (assessed and adjusted at the 4-week review). Infection (rare, sterile technique), and a blocked blood vessel. The serious risk, minimised by cannula, checking for blood vessels, small deposits, reversible HA and hyaluronidase prepared in the room. Warning signs are skin blanching or mottling, disproportionate pain or any change in vision; Dr Nish provides his direct number and these require same-day assessment.
