MEDICAL MICRONEEDLING · MAYFAIR & MARYLEBONE, LONDON

Dermapen 4 Microneedling London – for Acne Scars, Pores & Texture, by Dr Nish Shah MD, Mayfair

Dermapen 4 microneedling uses an FDA-cleared, 16-needle medical device to create up to 1,920 controlled micro-channels a second in the skin, switching on your skin’s own collagen and elastin production — smoothing acne scars, tightening pores, refining texture and softening fine lines without heat, chemicals or filler. The device is the same in every clinic. What is not the same is who decides the needle depth for each part of your face, and that is where results are made or lost. At Medisha Clinic every Dermapen 4 course is doctor-led: Dr Nish Shah MD, a medically trained doctor with over 30,000 non-surgical treatments, assesses your skin and writes the depth plan for every zone of your face; a trained aesthetic nurse delivers it under his direction; and he reviews your result. Results build over 4–12 weeks and keep improving for months after a course. Suitable for fair to deep skin tones.

Dermapen 4 — FDA-cleared (510(k) K221070) for facial acne scars, with a CE Declaration of Conformity; 16 needles, 0.2–3.0 mm

Doctor-led — assessed and depth-planned by Dr Nish Shah MD, over 30,000 non-surgical treatments; delivered by a trained aesthetic nurse under his direction

Depth planned zone by zone, from 0.2 mm to 3.0 mm — the decision that separates a result from a red face

Sterile, single-use cartridge for every patient, with a chip that stops it ever being used again and valves that stop fluid tracking back into the device — nothing that touched another face touches yours

Acne scars, enlarged pores, uneven texture, fine lines, pigmentation, surgical scars and stretch marks — suited to darker skin where many lasers are not

Redness for 24–48 hours, then back to daily life; results from 4 weeks, building for months; £50 consultation redeemable against treatment

The Treatment

What Is Dermapen 4 Microneedling?

Microneedling — also called collagen induction therapy — uses very fine needles to make thousands of tiny, controlled channels in the skin. The skin treats each one as an injury to repair, and repairs it by building new collagen and elastin. Repeated over a course, that renewal smooths acne scars, tightens enlarged pores, evens out texture and tone and softens fine lines — using nothing but your own skin’s response. No heat, no chemicals, nothing injected.

Dermapen 4 is the device Medisha uses, and it matters which device is used. Dermapen 4 is cleared by the US FDA (510(k) K221070, December 2022) to improve the appearance of facial acne scars in adults aged 22 and over, in Fitzpatrick skin types I to V, and carries a CE Declaration of Conformity. Each cartridge holds 16 ultra-fine 33-gauge surgical steel needles that stamp vertically at 80–110 times a second, up to 1,920 micro-channels a second, so the device glides rather than drags. Every cartridge is sterile and single-use, with a chip that prevents it ever being used again and retention valves that stop fluid tracking back into the handpiece. The needle depth adjusts from 0.2 mm to 3.0 mm, including a dedicated scar setting at full depth, and that adjustability is the whole point.

Because the skin of the forehead, the cheek, the nose, the area around the eye and an acne scar are all different thicknesses, one depth for the whole face is wrong for most of it. Too shallow and nothing happens; too deep in thin skin and you get bruising and pigmentation for no benefit. Choosing the right depth for each zone — and adjusting it as your skin responds over a course — is a medical judgement. At Medisha it is made by Dr Nish Shah MD, a medically trained doctor with over 30,000 non-surgical treatments, and carried out to his plan by a trained aesthetic nurse.

The Technology

Why Dermapen 4 — And Why the Depth Plan Matters More

Not all microneedling is equal, and the device is only half of it. Dermapen 4 gives the clinic a cleared, sterile, precisely adjustable instrument. The depth plan Dr Nish writes for your face — and the nurse follows — is what you are actually paying for.

  • FDA-cleared, with its own clinical data. Cleared by the US FDA in December 2022 (510(k) K221070) to improve facial acne scars in adults 22 and over, in skin types I to V, on 2 clinical studies in which 70–85% of patients were rated improved after a short course. It carries a CE Declaration of Conformity. Most microneedling pens on the market carry no medical clearance at all.
  • Sterile, single-use, impossible to reuse. A new sterile 16-needle cartridge for every patient, with a chip that locks it out once it is removed and retention valves that stop fluid flowing back into the handpiece. Cross-contamination — the real risk with reused or poorly cleaned pens — is designed out.
  • Needle depth from 0.2 mm to 3.0 mm, with a dedicated scar setting. Depth accuracy was tested as part of the FDA clearance, so the depth on the screen is the depth the needles reach, and the 3.0 mm scar setting reaches deep atrophic scars and stretch marks that shallower pens cannot. Cheaper devices are not held to that, which is how ‘1.5 mm’ ends up meaning anything.
  • Depth planned zone by zone by a doctor. Dr Nish maps your face before the first session: shallower over the forehead and around the eyes where skin is thin, deeper through cheek acne scarring where the collagen needs reaching, adjusted at each review as your skin responds. Our aesthetic nurse treats to that map. This is where the result is decided.
  • Suited to darker skin. Because Dermapen 4 works mechanically, without heat, it does not carry the pigmentation risk that limits many lasers and peels in darker skin: in a randomised trial, microneedling caused no post-inflammatory pigmentation where a fractional laser caused it in 13.6% of patients. The FDA clearance covers skin types I to V; Dr Nish assesses deeper skin tones individually.

SKIN TREATMENTS COMPARED

Dermapen 4 Microneedling vs Skin Boosters vs Polynucleotides vs Laser & Peels

Texture, scars, pores and dull skin bring patients to four different treatments, and clinics tend to recommend whichever they own. Dr Nish performs the first 3 and will tell you which your skin needs — and it is often a combination.

Dermapen 4 Microneedling — At Medisha

  • Mechanically triggers your own collagen renewal
  • Suited to acne scars, pores, texture and fine lines
  • Course of 3–6, 4 weeks apart; results build for months
  • 24–48 hours of redness; suited to fair to deep skin tones
  • Doctor-planned, nurse-delivered

Profhilo / Sunekos Performa

  • Injected bio-remodelling of the skin itself
  • Suited to firmness, crepiness, hydration and glow
  • 2–4 sessions; 6–9 months
  • Minimal downtime
  • Also by Dr Nish — the ideal partner to microneedling

Polynucleotides

  • Injected repair signals for damaged or inflamed skin
  • Suited to under-eyes, redness-prone and post-acne skin
  • 3–4 sessions; up to 12 months
  • Minimal downtime
  • Also by Dr Nish

Laser & Chemical Peels

  • Heat or acid removes or remodels the surface
  • Suited to pigmentation and sun damage in lighter skin
  • 1–3 sessions
  • Days to a week of peeling; pigmentation risk in darker skin
  • Not offered at Medisha; referred where appropriate

The most common Medisha programme for scarred or textured skin is a Dermapen 4 course to rebuild the surface, with Profhilo or Sunekos Performa to improve the skin’s quality and glow between or after sessions. Dr Nish sets out the sequence at consultation.

WHAT IT TREATS

What Dermapen 4 Microneedling Treats at Medisha Clinic

Face, neck, décolletage and body scars. Each concern has its own depth plan and course length, agreed at consultation.

Acne Scars

The indication Dermapen 4 is FDA-cleared for. Rolling and boxcar scars soften as new collagen fills the depression from beneath. The dedicated scar setting reaches 3.0 mm through the scar, shallower around it. Typically 4–6 sessions.

Enlarged Pores

Pores look smaller as the collagen around them tightens and the surface refines. One of the fastest visible changes, often from the second session.

Uneven Texture & Dull Skin

Rough, bumpy or tired-looking skin becomes smoother and brighter as the surface is renewed. The ‘glass skin’ request.

Fine Lines & Early Wrinkles

Fine lines around the eyes, mouth and forehead soften as the skin thickens with new collagen. For expression lines that move, anti-wrinkle injections do the other half.

Pigmentation & Sun Damage

Uneven tone and sun-related marking improve as the skin turns over — without the pigmentation risk heat-based treatments carry in darker skin.

Surgical Scars & Stretch Marks

Caesarean, surgical and injury scars and stretch marks on the body soften and flatten over a course. Older scars respond; newer ones respond faster.

Suitability

Am I A Good Candidate For Dermapen 4 Microneedling?

Dermapen 4 suits almost every skin tone and most skin concerns where the problem is the surface and the collagen beneath it. Dr Nish assesses your skin, your history and what you are trying to change before recommending a course.

Good Candidates

  • Acne scarring — rolling, boxcar or generally pitted (adults 22 and over, per the device clearance)
  • Enlarged pores or rough, uneven texture
  • Fine lines and early skin ageing
  • Dull, tired-looking skin that skincare has not lifted
  • Pigmentation or sun damage, including in darker skin
  • Surgical scars, caesarean scars or stretch marks
  • Wanting a natural, no-filler improvement in skin quality

May Not Be Suitable

  • Active acne, cold sores or any infection in the area — treated once clear
  • Isotretinoin (oral acne medication) in the last 6 months
  • A tendency to keloid scarring (raised, overgrown scars)
  • Recent sunburn or a fresh tan in the area
  • Pregnancy or breastfeeding
  • Sagging or lost volume — microneedling will not lift or fill; threads or filler instead, and Dr Nish will say so

The only way to know for certain is a consultation with Dr Nish. He will assess your anatomy and goals in person and give you an honest recommendation. The consultation is £50, redeemable in full against treatment, with no obligation to proceed.

Step by step

The Dermapen 4 Microneedling Process

Every Dermapen 4 course at Medisha follows the same medically supervised protocol: planned and reviewed by Dr Nish, delivered by a trained aesthetic nurse.

01

Consultation & Depth Map with Dr Nish

Dr Nish examines your skin under magnification, identifies what is scar, what is pore and what is texture, and writes the depth plan for each zone of your face. Your course length is agreed and your nurse is briefed. £50, redeemable against treatment.

02

Preparation

Stop retinoids, acids and exfoliants 3–5 days beforehand. Arrive make-up free. Skin is cleansed and photographed under standardised light.

03

Numbing

Topical anaesthetic for 20–30 minutes, then removed and the skin cleansed again. Most patients describe the treatment as a light vibrating scratch.

04

Dermapen 4 Treatment

A new sterile 16-needle cartridge is opened in front of you and clicks into the handpiece, which reads its chip. Our aesthetic nurse passes the device over each zone at the depth Dr Nish has planned, from 0.2 mm around the eyes to the 3.0 mm scar setting through scarring. 30–45 minutes for the face.

05

Immediately After

Skin is pink to red, like mild sunburn, and feels warm and tight. A soothing post-treatment protocol is applied. No make-up for 24 hours. You go home with written aftercare.

06

Course & Review

Sessions are 4 weeks apart — the time new collagen takes to form. Dr Nish reviews your progress and adjusts the depth map as your skin responds. Results build for up to 6 months after the final session.

Contact Dr Nish if redness worsens after 48 hours or you see any sign of infection.

Do

  • Use only the post-treatment products advised for the first 24 hours
  • Apply SPF 50 every morning — the single most important step
  • Sleep on a clean pillowcase for the first 2 nights
  • Drink water and keep the skin moisturised
  • Wait 24 hours before make-up
  • Attend every session 4 weeks apart

Avoid for 3–5 days

  • Retinoids, acids, vitamin C and any exfoliant
  • Make-up for 24 hours
  • Direct sun, sunbeds, saunas and hot yoga
  • Swimming pools and the gym for 48 hours
  • Touching, picking or scrubbing the skin
  • Other facial treatments for 2 weeks

Clinical Evidence

What the Research Shows

Microneedling is one of the more studied procedures in aesthetic dermatology, and Dermapen 4 carries an FDA clearance with clinical data of its own.

US FDA 510(k) K221070, December 2022

The FDA cleared the Dermapen 4 (DP4) microneedling device to improve the appearance of facial acne scars in adults aged 22 and over, in Fitzpatrick skin types I to V. In the first supporting study, 22 patients had 2 or 3 treatments 4 weeks apart and 77% were graded improved by blinded evaluators, with no patient worse; in the second, 20 patients had 3 treatments and 70–85% were rated improved, with side effects limited to transient redness, dryness and peeling. FDA 510(k) summary K221070, decision 20 December 2022

JAMA Dermatology, 2014

A randomised split-face trial of microneedling for facial acne scars, with one side of the face treated and the other left untreated: blinded dermatologists found scar scores significantly lower than baseline on the treated side at 6 months, with no change on the untreated side and no withdrawals for adverse events. Alam M, Han S, Pongprutthipan M, et al. JAMA Dermatol 2014;150(8):844–849

International Journal of Dermatology, 2015

Ten patients had 6 microneedling sessions 2 weeks apart; skin samples afterwards showed significant increases in collagen types I, III and VII and in newly formed collagen and elastic fibres compared with baseline, the objective evidence for the collagen response behind the treatment. Honest About What It Won’t Do

Microneedling does not lift, fill or replace lost volume. If your skin needs threads, filler or an injectable instead of — or as well as — Dermapen 4, Dr Nish performs those too and will say so.

Dermapen 4 microneedling is medical microneedling using the Dermapen 4 device, which is cleared by the US FDA for facial acne scars (510(k) K221070, 2022). A sterile cartridge of 16 ultra-fine needles stamps vertically at 80–110 times a second, making up to 1,920 controlled, microscopic channels a second in the skin. The skin treats each channel as a tiny injury and repairs it by producing new collagen and elastin. Over a course of sessions that renewal smooths acne scars, tightens enlarged pores, refines texture, softens fine lines and evens tone, using only your skin’s own response. There is no heat, no chemical and nothing injected. The needle depth is adjustable from 0.2 mm to 3.0 mm, and at Medisha Clinic the depth for each zone of your face is planned by Dr Nish Shah MD and delivered by a trained aesthetic nurse to his plan.

At Medisha Clinic in Mayfair, Dermapen 4 is priced per session and per course, with course prices confirmed at your consultation. Every session uses a new sterile Dermapen 4 cartridge, opened in front of you, and is delivered by a trained aesthetic nurse to the depth plan Dr Nish Shah MD writes for you at consultation. A typical face course is 3 sessions for texture, pores and fine lines, and up to 6 sessions for acne scarring, 4 weeks apart. Consultation with Dr Nish is £50, redeemed in full against treatment. The clinic is at 58 South Molton Street, Mayfair, London W1K 5SL, 3 minutes from Bond Street station.

Dermapen 4 differs from most microneedling in 3 ways: medical clearance, sterility and depth. Clearance: it is cleared by the US FDA for facial acne scars in adults 22 and over (510(k) K221070), on clinical studies in which 70–85% of patients were rated improved, and it carries a CE Declaration of Conformity; most microneedling pens carry no medical clearance at all. Sterility: every patient gets a new sealed 16-needle cartridge with a chip that stops it ever being used again, and retention valves stop fluid flowing back into the handpiece, so cross-contamination, the real risk with reused or poorly cleaned pens, is designed out. Depth: 0.2 mm to 3.0 mm, with depth accuracy tested as part of the clearance and a dedicated scar setting at full depth. The device is only half the story. At Medisha the depth for each part of your face is chosen and adjusted by a medically trained doctor with over 30,000 non-surgical treatments, and delivered by a trained nurse to that plan.

Yes, and this is one of its real advantages. Many lasers and chemical peels work with heat or acid, which can trigger dark marks (post-inflammatory pigmentation) in darker skin. Dermapen 4 works mechanically, without heat. In a randomised trial comparing microneedling with a fractional laser for acne scars, no microneedling patient developed post-inflammatory pigmentation, against 13.6% of laser patients. The FDA clearance covers Fitzpatrick skin types I to V; Dr Nish Shah MD assesses the deepest skin tones individually, as he does every patient’s history of pigmentation, at consultation. Strict sun protection after treatment is the other half of keeping darker skin safe, and it is part of your aftercare at Medisha Clinic.

A numbing cream is applied for 20–30 minutes first. Most patients then describe Dermapen 4 as a light, vibrating scratch, most noticeable over the forehead and around the nose where the skin is thin; because the 16 needles stamp vertically rather than drag, the device glides. Afterwards the skin is pink to red like mild sunburn, warm and tight, for 24–48 hours, with light flaking sometimes on days 2–4. You can return to work the next day and wear make-up after 24 hours. Pinpoint bruising is uncommon. For 3–5 days you avoid retinoids, acids, sun, saunas and the gym, and you wear SPF 50 every morning. Dr Nish Shah MD gives you written aftercare before you leave Medisha Clinic.

Most patients need a course of 3–6 sessions, 4 weeks apart, with results building for months afterwards. For texture, pores, dullness and fine lines, 3 sessions 4 weeks apart is typical. For acne scarring, 4–6. The gap matters: 4 weeks is roughly how long new collagen takes to form, so treating sooner adds nothing. Skin often looks brighter within days of each session. The real change, new collagen, appears from about 4 weeks and keeps building for up to 6 months after the final session, so the most representative photographs are taken months after the course, not days. Dr Nish Shah MD reviews your progress at Medisha Clinic and adjusts the depth plan as your skin responds. A maintenance session every 6–12 months holds the result.

Yes. Facial acne scars in adults aged 22 and over are the indication Dermapen 4 is FDA-cleared for, on 2 clinical studies in which 70–85% of patients were rated improved after 2 or 3 treatments 4 weeks apart, and none worse. Rolling and boxcar scars, the shallow, soft-edged and box-shaped depressions, respond most strongly, because new collagen builds up beneath the scar and lifts its floor. Deep ice-pick scars respond less and may need other approaches. The depth plan is what makes the difference: the dedicated scar setting reaches 3.0 mm through the scar tissue where the collagen needs reaching, with shallower settings around it where the skin is normal, adjusted session by session by Dr Nish Shah MD. Active acne must be clear before treatment, and anyone who has taken isotretinoin in the last 6 months waits until that window has passed.

They work in different ways and are often combined. Dermapen 4 works from the surface: it mechanically triggers your skin to rebuild collagen where the needles have been, so it is the treatment for scars, pores, texture and fine lines. Profhilo and Sunekos Performa are injected and bio-remodel the skin from within, so they are the treatments for firmness, crepiness, hydration and glow. Polynucleotides are injected repair signals for damaged, inflamed or under-eye skin. Dr Nish Shah MD performs all of them at Medisha Clinic, so the recommendation follows your skin rather than the clinic’s equipment. The most common combination for scarred or textured skin is a Dermapen 4 course with Profhilo or Sunekos Performa between or after sessions.

Yes. The neck and chest are treated for crepiness, fine lines and sun damage at shallower settings than the face, because the skin is thinner, and the manufacturer’s protocols cover the face, neck, décolletage, hands and body. Surgical scars, caesarean scars, injury scars and stretch marks are treated over a course using the deeper scar setting, and older scars respond as well as newer ones, though newer ones respond faster. The FDA clearance itself is for facial acne scars; treatment of other areas is a clinical decision Dr Nish Shah MD makes for you at your Medisha Clinic consultation.

Anyone with active acne, cold sores, eczema, psoriasis or any infection in the treatment area should wait until it is clear. Anyone who has taken isotretinoin (oral acne medication) in the last 6 months waits until that window has passed. A history of keloid scarring (raised, overgrown scars), a fresh tan or sunburn, and pregnancy or breastfeeding are all reasons to postpone or avoid treatment. Microneedling also will not lift sagging skin or replace lost volume, so if that is the real problem, threads or filler are the right treatment and Dr Nish Shah MD will tell you so at your Medisha Clinic consultation.

In a medical setting with a sterile single-use device the risks are low. Expected, temporary effects: redness like mild sunburn for 24–48 hours, warmth and tightness, light flaking on days 2–4, and occasionally pinpoint bruising. Uncommon risks are infection, avoided by the sealed, chip-locked cartridge and clean aftercare, and a cold sore flare in people who get them, for which a preventive can be prescribed. Temporary darkening of the skin can occur if the area is exposed to sun too soon, which strict SPF avoids. With poor technique or excessive depth, track marks or scarring can occur, which is why the depth is set by a doctor. Depth judgement and sterility are the two things that decide safety, which is why every Medisha Clinic course is planned and reviewed by Dr Nish Shah MD and delivered with a new cartridge opened in front of you.

Because the device is the same in every clinic that owns one, and the result is decided by who plans the depth for your face. At Medisha that is Dr Nish Shah MD: a medically trained doctor with over 30,000 non-surgical treatments, one of the most experienced aesthetic doctors in the UK and London, Senior International Clinical Trainer at Derma Medical, Official UK Trainer for Sunekos, and Brand Ambassador and Key Opinion Leader for Intraline. He maps your depth zone by zone at consultation; a trained aesthetic nurse delivers each session to that plan with a new sterile cartridge opened in front of you; and he reviews and adjusts the plan as your skin responds. Because he also performs Profhilo, Sunekos, polynucleotides, threads and filler, you get the treatment your skin needs rather than the one the clinic owns, and you will be told plainly if microneedling is not it. £50 consultation redeemed in full against treatment. 58 South Molton Street, Mayfair, London W1K 5SL, serving Mayfair and Marylebone.
