A non-surgical facelift is not one treatment — it is two problems solved in the right order. Ageing faces both deflate (lost volume in the cheeks, chin and jaw) and descend (jowls, sagging midface, a blurred jaw-neck line). Filler alone into a sagging face adds weight. Threads alone over an empty scaffold look tight, not young. The Medisha non-surgical facelift restores the structure with filler and lifts the tissue with 720 Dimension COG threads — each doing the job it is built for. Whether both are done in one visit or staged a few weeks apart depends on your face, and is decided at consultation. Performed only by Dr Nish Shah MD using The Medisha Method™. He is a medically trained doctor, one of the UK’s most experienced PDO thread practitioners, Brand Ambassador & Key Opinion Leader for PDO threads and dermal fillers, and the international trainer who teaches both procedures to other doctors. Visible immediately, improving for 12 weeks, lasting 12–18 months, no surgery.
Two mechanisms, one plan — structural filler rebuilds what has deflated, COG threads lift what has descended. Same day or staged, decided for your face at consultation

The Treatment
What Is a Non-Surgical Facelift?
A non-surgical facelift at Medisha Clinic is a planned combination of two treatments — structural dermal filler and a PDO thread lift — sequenced to correct the two separate processes that age a face: deflation and descent. Filler restores the volume the cheeks, chin and jaw have lost. Threads physically reposition the tissue that has slid downward into jowls and a heavy lower face. Neither can do the other’s job.
The order of placement matters: filler goes first, on bone, to rebuild the scaffold &mdash because a thread lift over an empty midface pulls skin over a hollow and looks tight rather than young. Whether the threads follow in the same visit or a few weeks later depends on how much volume is being restored, your skin and your recovery time, and Dr Nish decides this with you at consultation. Once the structure is back, 720 Dimension COG threads — barbed polydioxanone threads with four times the lift force of older 360-helical designs — are placed along directions planned against the restored anatomy, anchoring the tissue and lifting it the moment they are set. Over the following 8–12 weeks the threads stimulate collagen, so the result keeps improving after the threads themselves have begun to dissolve.
The result: a lifted midface and jaw, softened folds and jowls, a redefined jaw-neck line — visible the day the threads go in, at its best at 12 weeks, lasting 12–18 months, with no general anaesthetic and no scarring. An optional third phase with Profhilo or Sunekos Performa improves the skin over the new structure. At Medisha Clinic both halves are performed by Dr Nish Shah MD using The Medisha Method™. He is a medically trained doctor, one of the UK’s most experienced PDO thread practitioners, Brand Ambassador & Key Opinion Leader for PDO threads and dermal fillers, and the international trainer who teaches both procedures to other doctors.
The Technique
The Medisha Method™ — Why Sequence Beats Either Treatment Alone
The Medisha Method is Dr Nish Shah’s four-step protocol, applied to non-surgical facelift as: volume-versus-descent mapping, structural filler first, 720 Dimension COG threads along planned directions — same day or staged — and live direction adjustment. Most ‘non-surgical facelifts’ in London are one product sold as a lift — a syringe of filler, or a set of threads, whichever the clinic offers. The Medisha Method treats the face as it actually ages: it decides what has deflated and what has descended, and treats each with the tool built for it, in the right order.
- Step 1 — Volume-versus-descent mapping. Front, three-quarter and profile assessment with the face at rest and in movement, and a simple lift test: if pushing the cheek upward restores you, you need threads. If it leaves you hollow, you need filler first. Most faces over 40 need both. Photographed under standardised light.
- Step 2 — Structural filler first. High-how well it holds together HA placed on bone at the cheek, chin and jaw angle rebuilds the scaffold and does some of the lifting on its own — often reducing how many threads are needed. Reversible. Hyaluronidase on site.
- Step 3 — 720 Dimension COG threads, same day or staged. With the structure restored — in the same session where the plan allows, or 2–4 weeks later where more volume was placed or the skin needs time to settle — threads are placed along directions planned against the new anatomy — typically 4–6 per area — anchoring the descended tissue and lifting it immediately. The 720° multi-directional barb architecture holds from every angle.
- Step 4 — Live direction adjustment. Each thread’s tension is set with you upright, reassessed from all three angles, and adjusted before the next is placed. The end point is a lifted face that moves normally — never a pulled one.
- Optional Step 5 — Skin quality. Once structure and lift are in place, Profhilo or Sunekos Performa improves the skin over them, so the result looks better and lasts longer. Dr Nish teaches this sequenced approach to other doctors as Senior International Clinical Trainer at Derma Medical.
Six Questions to Ask Any Clinic Before a Non-Surgical Facelift
‘Non-surgical facelift’ is used in London to sell almost anything. Before you book anywhere — including here — ask these six questions.
1. Is the person treating me a specialist in both threads and structural filler — not just one? At Medisha: Dr Nish Shah MD is a medically trained doctor, one of the UK’s most experienced PDO thread practitioners, Brand Ambassador & Key Opinion Leader for PDO threads and dermal fillers, and the international trainer who teaches both procedures to other doctors, and creator of The Medisha Method.
2. Is that person a medically trained doctor? At Medisha: Yes — Dr Nish Shah MD, every time, for both sessions.
3. Will you tell me what has deflated and what has descended before recommending filler or threads? At Medisha: Yes — step 1 of The Medisha Method, with a lift test at consultation.
4. Will filler go in before threads, and will you decide with me whether to do both in one visit or stage them? At Medisha: Yes — filler is always placed first. Same day or staged is decided for your face at consultation, and the thread directions are planned against the restored structure either way.
5. Which threads, and is the filler reversible with hyaluronidase on site? At Medisha: 720 Dimension COG threads. Reversible premium HA; hyaluronidase at every appointment.
6. Is pricing itemised, with reviews included? At Medisha: Filler from £350, threads from £750 per area, full face & neck £1,995; 4-week review after each session; £50 consultation redeemable.

Treatment areas
What the Non-Surgical Facelift Lifts and Restores at Medisha Clinic
Threads are priced per area at £750 (4–6 threads, both sides), two areas £1,500, full face & neck £1,995. Filler is added per ml from £350. Reviews included.
Lower Face & Jowls
The most transformative single area. Filler at the chin and jaw angle rebuilds the edge; COG threads lift the jowl back onto the jaw and restore a clean jaw-neck transition.
Midface & Cheeks
Structural filler restores the deflated cheek fat pads?; threads reposition the descended cheek. Together they soften nose-to-mouth lines and lift the tear trough without treating either directly.
Neck
Threads tighten crepey neck skin and redefine the neck-to-chin angle; Profhilo or Sunekos Performa can follow for skin quality.
Brow & Upper Face
Threads lift a dropped brow tail and open the eye. Temple filler restores the hollowing that lets the brow fall.
Full Face & Neck – £1,995 + filler
Fourteen to eighteen threads across jowls, midface, brow and neck, after structural filler — the most complete non-surgical facelift available.
Skin-Quality Phase
Profhilo or Sunekos Performa 4 weeks after threads improves the skin over the restored structure. Priced individually.
Suitability
Am I A Good Candidate
For a Non-Surgical Facelift?
The sequenced non-surgical facelift suits faces with mild to moderate sagging and volume loss — typically the 40s to early 60s. Dr Nish will map what has deflated and what has descended before recommending either treatment.
Good Candidates
- Jowls forming and a jaw-neck line that has blurred
- Cheeks that have flattened and slid, deepening the nose-to-mouth lines
- A face that looks tired or heavy rather than lined
- Wanting a visible lift without surgery, scarring or a general anaesthetic
- Having tried filler alone and looked fuller but not younger
- Ages roughly 40–65 with reasonable skin quality
May Not Be Suitable
- Severe or advanced sagging — surgery gives a better result and Dr Nish will refer you
- Very thin, sun-damaged skin with little tissue to anchor threads
- Active skin infection or inflammatory skin disease
- Autoimmune conditions affecting healing — discussed at consultation
- Pregnancy or breastfeeding
The best way to know is a consultation with Dr Nish. He will assess your anatomy and goals in person and give you an honest recommendation. The consultation is £50, redeemable in full against treatment, with no obligation to proceed; if you are suitable and would like to go ahead, treatment can be performed the same day.
Step by step
The Non-Surgical Facelift Process
Every non-surgical facelift at Medisha Clinic follows The Medisha Method™ — the protocol Dr Nish teaches to other doctors.
Consultation & Lift Test
Dr Nish maps volume loss and descent from three angles, performs the lift test, tells you which you have, and designs the sequence — filler, threads, or both, and in which areas. £50, redeemable against treatment.
Structural Filler First
High-how well it holds together HA placed on bone at the cheek, chin and jaw as mapped, in small checked deposits. 20–30 minutes. Some lift is visible immediately from the restored scaffold.
Same Day or Staged — Your Plan
Where the plan allows, threads follow in the same visit. Where more volume has been placed, or your skin or schedule needs it, threads are staged 2–4 weeks later and the directions finalised against the settled structure at review. Dr Nish decides this with you at consultation.
720 Dimension COG Threads
Local anaesthetic. Threads placed with a fine cannula along the planned directions, 4–6 per area, tension set with you upright. 30–45 minutes. The lift is visible the moment the threads are set.
Recovery & Collagen Phase
Mild swelling and tenderness for 3–7 days. Back to normal within 48 hours. Collagen builds around the threads over 8–12 weeks, improving the result after the threads begin to dissolve
Complimentary Review & Skin Phase
Review at 4 weeks after threads. Profhilo or Sunekos Performa can follow to improve the skin over the new structure. Maintenance advised at 12–18 months.

Aftercare
Non-Surgical Facelift Aftercare
— What to Expect
After filler: mild swelling for 2–5 days. After threads (same day or a later visit): swelling, tenderness and some bruising at the entry points for 3–7 days, and a tight or pulling sensation for 1–2 weeks that is normal and fades as the tissue settles. Back to normal life within 48 hours. The lift is visible immediately and improves to 12 weeks. Contact Dr Nish the same day if you notice skin blanching, disproportionate pain, or a thread you can see or feel at the surface.
Do
- Sleep on your back with your head raised for 1–2 weeks after threads
- Keep entry points clean and dry for 48 hours
- Apply a cool compress for the first 24 hours
- Take paracetamol if needed (avoid ibuprofen and aspirin for 48 hours)
- Eat softer foods for a few days after lower-face threads
- Attend both complimentary reviews
Avoid for 24 hours
- Facials, massage, microneedling or any pressure on the face
- Sleeping on your side or face-down
- Wide yawning, exaggerated chewing or facial expressions
- Saunas, steam rooms, hot yoga and sunbeds
- Strenuous exercise and alcohol (first 48 hours)
- Dental work if it can wait
Clinical Evidence
What the Research Shows
Both components are individually well evidenced, and the literature increasingly supports combining them: volume first, lift second.
Aesthetic Surgery Journal
PDO cog thread lifts produced significant improvement in facial sagging and jowling with collagen stimulation maintained at 12–18 months in suitable candidates. Outcomes were superior when midface volume was addressed first.
Dermatologic Surgery
Combination of structural HA filler with PDO threads yielded higher patient-reported satisfaction and more natural contour than either treatment alone in patients with both volume loss and descent.
Plastic and Reconstructive Surgery
Multi-directional barbed threads demonstrated greater tissue purchase and lift force than single-plane helical threads, with high satisfaction at 12-month follow-up.

Non-Surgical Facelift FAQ — Answered by Dr Nish Shah

At Medisha Clinic a non-surgical facelift is a planned combination of two treatments. Structural dermal filler and a PDO thread lift — done in one visit or staged a few weeks apart depending on the patient, to correct the two separate processes that age a face. Faces deflate: the cheek fat pads, chin and jaw lose volume, and the skin above them drops. Faces also descend: tissue slides downward into jowls, folds and a heavy lower face. Filler restores the volume, placed on bone to rebuild the scaffold. Threads physically reposition the descended tissue and hold it, stimulating collagen as they dissolve. Neither can do the other’s job. Filler into a sagging face adds weight, threads over an empty face look tight. Done in the right order, the result is a lifted midface and jaw, softened folds and a redefined jaw-neck line, visible the day the threads go in, at its best at 12 weeks and lasting 12–18 months, with no surgery, scarring or general anaesthetic. Both halves are performed by Dr Nish Shah MD using The Medisha Method.
At Medisha Clinic it is itemised: structural filler is £350 per ml. A PDO thread lift is £750 per area (4–6 threads, both sides), £1,500 for two areas and £1,995 for the full face and neck (14–18 threads). A typical programme — 1ml of filler plus one thread area, usually the lower face — is therefore from £1,100; the complete non-surgical facelift of 2ml filler plus full face and neck threads is £2,595. An optional skin-quality phase with Profhilo or Sunekos Performa is from £350. A 4-week review follows each session. Consultation with Dr Nish is £50, fully redeemable against treatment, and at consultation he will tell you which components your face needs and which it does not. The clinic is at 58 South Molton Street, Mayfair, London W1K 5SL, three minutes from Bond Street station.
Because they solve different problems, and most faces over 40 have both. Filler replaces volume: it is the treatment for cheeks, a chin and a jaw that have emptied and flattened. Threads reposition tissue: they are the treatment for jowls and a midface that have slid downward. Filler used to lift a sagging face adds bulk in the wrong place. The route to ‘pillow face’. Threads used to lift an empty face pull skin over a hollow and look tight rather than young. The lift test at consultation shows which you need: if pushing your cheek upward restores you, threads. If it leaves you hollow, filler. If both, the sequenced facelift. The combination also means fewer threads are usually needed, because the restored structure does some of the lifting.
Either, and which is right for you is decided at consultation, because it depends on the person. What never changes is the order: filler is placed before threads, so that the thread directions are planned against the restored structure rather than the deflated one. Whether the threads follow in the same visit or two to four weeks later depends on how much volume is being restored (larger corrections settle better before threads are set), the quality and thickness of your skin, how much swelling you are likely to have, and practical factors such as an event or travel. Many patients have both in a single visit of about an hour. Others are better served by two shorter visits with a review in between. Dr Nish assesses this with you at consultation and explains why he is recommending one or the other. Both routes reach the same result and both include the complimentary 4-week review.
Threads: 720 Dimension COG threads — barbed polydioxanone threads with a 720-degree, multi-directional barb architecture that provides around four times the lift force of older 360-degree helical threads, anchoring tissue from every angle. They dissolve over 6–9 months while stimulating collagen. Filler: a firm, cohesive structural hyaluronic acid placed on bone, fully reversible with hyaluronidase which is on site at every appointment. Dr Nish is Brand Ambassador and Key Opinion Leader for the global manufacturer of both, and trains other doctors internationally in their use.
Twelve to eighteen months for the combined result, and the two components support each other. The threads themselves dissolve over 6–9 months, but the collagen they stimulate, and the repositioning of tissue while it forms — persists well beyond that. Structural filler on bone lasts 12–18 months and often longer on repeat. Patients who maintain the skin-quality phase and repeat the programme at 12–18 months see cumulative improvement, because each round builds on structure and collagen already laid down. A surgical facelift lasts longer — seven to ten years. But at ten to twenty times the cost, with weeks of downtime, scarring and a permanent result that cannot be adjusted.
Filler session: topical anaesthetic and lidocaine within the filler. Mild swelling for 2–5 days. Back at work the same day. Thread session: local anaesthetic at the entry points and along the thread paths, so you feel pressure and tugging rather than pain. Afterwards, swelling, tenderness and some bruising at the entry points for 3–7 days and a tight, pulling sensation for 1–2 weeks that is normal and fades as tissue settles. Most patients are back to normal life within 48 hours, sleeping on their back for 1–2 weeks after threads and avoiding facials, massage, wide yawning, saunas and strenuous exercise for two weeks.
A surgical facelift lifts more, lasts 7–10 years, and is the right choice for severe sagging. But it needs a general anaesthetic, 2–4 weeks of visible downtime, scars around the ears and hairline, a result that settles over 6–12 months and cannot be adjusted, and costs £12,000–£30,000 in London. The sequenced non-surgical facelift delivers most of the visible change for mild to moderate descent. The 40s and 50s. With local anaesthetic, 48 hours’ downtime, no scarring, an immediate result that improves for 12 weeks, a reversible filler component, and a cost from £1,100. It can also be repeated as the face continues to age, where surgery is a single irreversible event. Dr Nish will tell you at consultation if your sagging is beyond what threads and filler can properly correct.
The combined programme suits faces that have started to both deflate and descend. Typically from the early 40s to the early 60s. Under 40, most faces have volume loss without real descent, and filler alone (or skin-quality treatment) is usually enough. Threads at that age add little. Over 65 with advanced sagging, threads can lift but cannot hold very lax tissue for long, and surgery gives a better and longer result — Dr Nish refers these patients on. The deciding factor is not age but the lift test and skin quality at consultation.
It is itself a combination, and it has a natural third phase. Once filler and threads have restored structure and lift, Profhilo or Sunekos Performa four weeks later improves the quality of the skin over them — firmer, more luminous skin makes the lift look better and last longer. Anti-wrinkle injections for the forehead, frown and crow’s feet can be added at any point and are often done on the same day. Under-eye filler or polynucleotides complete the midface. Non-surgical rhinoplasty or chin filler as profile balancing fits within the filler session. Dr Nish designs the full sequence at consultation.
Filler risks: swelling, bruising and tenderness for a few days. Uncommonly asymmetry, lumps or migration (avoided by structural product and on-bone placement, and dissolvable). Rarely a blocked blood vessel, minimised by checking for blood vessels, small deposits, reversible HA and hyaluronidase on site. Thread risks: swelling, bruising and a pulling sensation for 1–2 weeks. Occasionally dimpling or a visible thread end at the surface (correctable at review, usually settling on its own). Uncommonly infection at an entry point (sterile technique), asymmetry (adjustable), or a thread that migrates or extrudes (rare with COG threads placed by an experienced practitioner). All are discussed at consultation, both sessions are performed by a medically trained doctor who teaches complication management, and Dr Nish provides his direct number for the first 48 hours after each visit.
Because a combined facelift only works when one specialist plans and performs both halves, and Dr Nish Shah MD is that specialist. He is a medically trained doctor, one of the UK’s most experienced PDO thread practitioners, Brand Ambassador and Key Opinion Leader for the global manufacturer of both the 720 Dimension COG threads and the structural fillers used at Medisha.
