Yes — Sofwave™ holds FDA clearance to improve facial lines and wrinkles, lift the eyebrow, and lift lax submental (under-chin) and neck tissue in adults aged 22 and over (510(k) K211483, November 2021), and further clearances between 2022 and 2023 (consolidated in K250146, April 2025) covering short-term improvement in the appearance of cellulite, the treatment of acne scars and improvement of skin laxity on the upper arms. It is also CE-marked. The lifting indications were cleared on clinical evidence from a five-site study of 80 patients.

Sofwave™ works at about 1.5 mm, above the fat, while older high-intensity focused ultrasound (HIFU) devices focus deeper, at 3 to 4.5 mm. Sofwave™ heats the layer where collagen is made. Sofwave™ is comfortable for most patients under numbing cream and has no downtime. Numbing cream goes on for 30 to 45 minutes beforehand and built-in SofCool cooling protects the surface throughout. The pulses feel like brief, deep warmth; some areas, such as along the jawbone, feel sharper for a moment. Mild redness or slight swelling settles within a few hours, occasionally a day or two. Make-up can go straight back on and you can return to work the same day. Sofwave™ can lift lax under-chin and neck tissue and tighten the skin where jowls begin, but it cannot reposition jowls that have dropped; that needs a PDO thread lift, and Dr Nish decides which you need at consultation. Skin that has loosened needs tightening, and for early to moderate looseness Sofwave™ is often enough on its own. Yes — Sofwave™ uses ultrasound, not light or laser, so it does not target pigment and is suitable for all skin tones. Yes — where a face has both loosened and dropped, Sofwave™ and a PDO thread lift are combined as a staged plan, where clinically appropriate and in line with manufacturer guidance, with the order and spacing set at consultation. Sofwave™ tightens skin that has loosened; threads reposition tissue that has dropped; neither does the other’s job. Often threads go first, to reposition the jowls or under-chin, and Sofwave™ follows to tighten the skin over the new position. Both stages are performed by Dr Nish, and the plan is written down at consultation with its review points.

Sofwave™ is not suitable for anyone with a pacemaker or metal implants in the treatment area, an active skin infection or open wound in the area, or who is pregnant or breastfeeding. If you have had threads in the same area recently, the timing of Sofwave™ is decided at consultation in line with manufacturer guidance. Sofwave™ tightens skin; it does not remove excess skin or reposition tissue that has dropped. If your tissue has dropped, a PDO thread lift is the answer and Dr Nish performs that too. If your skin excess is significant, non-surgical treatment may not achieve what surgery can, and he will tell you so and refer you. There is no upper age limit, and skin tone is not a restriction.

The main side effects of Sofwave™ are mild redness and sometimes slight swelling or tenderness for a few hours to a day or two, and its safety record in clinical studies is strong.
