A jowl is one of two things: skin that has gone slack along the jaw, or a pocket of tissue that has slid below it. Sofwave™ tightens the first. 720 PDO threads lift the second. Some jawlines have done both and need the two in sequence. A few are surgical, and are told so. Dr Nish Shah MD settles which yours is at the mirror, before anything is recommended.
1 · Tightens — Sofwave™
– The jaw edge has blurred; no pocket yet
– Single visit, straight back to your day
– Edge firms from week 12 to month 6
2 · Lifts — 720 PDO Threads
– A pocket sits below the jaw
– Drawn back along the jaw while you watch
– Holds for 12–18 months
1+2 · Both, in sequence — Threads First, Sofwave™ After
– Pocket dropped, skin over it slack
– Threads first; ultrasound at the gap Dr Nish sets
– The usual picture past 45, in Dr Nish’s experience
More loose skin than threads or ultrasound can gather? That is a surgical case. You hear it from Dr Nish at the assessment, not after a treatment that did not show.
The mirror first. Dr Nish lifts, pinches and shows you whether your jowl is slack skin, a dropped pocket, or both
Ultrasound with a lifting clearance. Sofwave™ is FDA-cleared to lift lax under-chin and neck tissue — the skin a jowl runs into
Chin and jaw edge checked. A short chin lets a jowl hang lower; reversible structure filler only where it changes what you see
Surgery named when it is the answer. Skin excess along the jaw and neck is a surgeon’s job, and Dr Nish tells you so
Same hands for both. Dr Nish maps the ultrasound and places the threads himself: thousands of thread lifts, 30,000+ treatments
Nothing cut, no general anaesthetic. Sofwave™: about 30 minutes, make-up straight back on. Threads: a quiet evening, then two careful weeks

The Treatment
Why Do I Have Jowls?
Jowls are tissue that has slid down to the jaw, skin that has loosened over it, or both. The ligaments that hold the cheek up loosen with age, and the cheek drifts down until it pools at the jawline. At the same time the skin loses collagen and stops snapping back.
That is why one treatment never fixes every jowl. Tightening cannot move a jowl that has dropped. Lifting cannot shrink loose skin. Filler pushed into a jowl adds weight.
So the first job is to find out which jowl you have. Dr Nish does that at your assessment, shows you in the mirror, and recommends Sofwave™, threads or both. If a weak chin or soft jaw edge is making the jowl show, that is treated too.
TWO JOBS
1. Sofwave™ Tightens. 2. Threads Lift. Which Do Your Jowls Need?
Dr Nish has both tools in the same room and uses both himself, so the only thing steering the recommendation is what your jawline is doing.
- 1. Slack skin, no pocket → Sofwave™. The line from ear to chin has blurred, but nothing hangs below it. Ultrasound warms the collagen layer about 1.5 mm down, in one visit, with nothing to recover from. You notice the edge firming from week 12, and it carries on to month 6.
- 2. A pocket below the jaw → 720 PDO threads. Tissue has slid off the cheek and settled at the jaw. Under local anaesthetic, dissolvable 720 Dimension COG threads draw it back up along the jawline. The change is there when you sit up, and it holds for 12–18 months.
- 3. Pocket and slack skin → staged. Threads put the tissue back; Sofwave™ then firms the skin over it. A staged plan, where clinically appropriate and in line with manufacturer guidance: sequenced by Dr Nish, with the order and the timing set at consultation.
- + Structure, only where it changes the result. A weak chin or soft jaw edge makes any jowl show more. Reversible jawline or chin filler gives the lifted tissue an edge to sit on. Added only when it will change what you see.
- 4. Too much skin → a surgeon. When the excess along the jaw and neck is more than threads or ultrasound can gather, surgery may achieve what they cannot. Dr Nish says that at the first visit and refers you, so you are not paying for a half-result.
Sofwave™ vs PDO Threads for Jowls

— or Beyond Non-Surgical?
Four jawlines, four different conversations. Which column is yours?
Early — Sofwave™
- Jaw edge looks soft in photos from below
- No pocket when you turn to profile
- One visit; firmer from week 12
- Tightening on its own usually does it
Established — PDO Threads
- A pocket either side of the chin
- A marionette fold starting beside the mouth
- Drawn up along the jaw on the day
- Chin or jaw filler only if the edge is weak
Both — Staged
- Pocket dropped, skin over it slack
- Blurred edge and a pocket, side-on
- Threads, then Sofwave™, sequenced by Dr Nish
- The usual picture past 45, in Dr Nish’s experience
Beyond Non-Surgical — Surgery

YOUR OPTIONS
Every Treatment for Jowls Under One Roof
One place, one doctor. Follow a link for the full page.
1. Sofwave™ Jowls & Jawline
For the jaw edge that has gone soft but still has no pocket. One visit; firmer by month 6.
2. 720 PDO Jowl Lift
For the pocket that has settled below the jaw. Drawn back up the same day; holds 12–18 months.
3. Staged Plan — Non-Surgical Facelift
For a pocket that has dropped with skin that has slackened over it. Threads first, Sofwave™ after.
+ Jawline & Chin Filler
Structure. A short chin or a blurred jaw edge lets a jowl hang lower. Reversible filler, only where it changes the result.
+ Non-Surgical Neck Lift
When the jowl runs on into a sagging under-chin or neck, the whole line is planned as one.
4. Surgical Referral
For skin excess beyond any of the above. A referral to a surgeon rather than a lift that cannot keep up.
Suitability
Can My Jowls Be Treated Without Surgery?
From a jaw edge that has only just blurred to a pocket that has sat there for years, most jowls have a non-surgical answer. The assessment names yours.
Good Candidates
- A jawline that looks soft in photos taken from below
- A pocket either side of the chin you can see in profile
- A jaw-to-neck line that has lost its angle
- A marionette fold starting beside the mouth and running into the jowl
- A short or weak chin that lets the jowl hang lower
- Preferring an ultrasound session or threads to an operation
- Any skin tone; the decision rests on the jawline, not your age
May Not Be Suitable
- Skin excess no thread can gather — surgery may do more, and Dr Nish says so
- Thin skin with little tissue under it, where a thread outline could show
- An active infection or breakout over the jawline
- Pregnancy or breastfeeding
- Anticoagulant medication not yet reviewed (threads, filler)
- A pacemaker or metal implant near the jaw (Sofwave™)
Nodding at two of these at once? That is normal; most jowls sit between the categories. Dr Nish settles it with you at the mirror. The consultation is £50, redeemable in full against treatment, and you are under no obligation to go ahead. Nothing is booked that will not change your jawline.

Jowls form when three things change along the jaw at once. The skin loses collagen and goes slack. The ligaments holding the cheek weaken, so the tissue slides down and pools as a pocket at the jawline. And the chin and jaw bone recede, leaving less edge for that tissue to drape over. The skin changes begin in the mid-30s; the pocket usually shows from the mid-40s. Weight loss, sun, smoking and genetics hurry all three along. Which change is doing most of the work decides whether your jowl needs Sofwave™, threads or both, and that is what Dr Nish works out with you at the mirror.
Yes — most jowls can be treated without surgery, provided the right change is treated. A jaw edge that has blurred but has no pocket below it is tightened with Sofwave™ ultrasound: one visit, no downtime, the edge firming from week 12. A pocket that has already settled below the jaw is drawn back up along the jawline with 720 PDO threads, under local anaesthetic. You see that change in the mirror before you leave, and it holds for 12 to 18 months. A pocket with slack skin over it needs the two in sequence. Where loose skin runs on down the neck, surgery may achieve what neither can, and Dr Nish tells you that at the first visit.
Ask yourself one question: is there a pocket? Turn side-on to a mirror, or look at a photo taken from below. If the jaw edge has gone soft but nothing hangs below it, Sofwave™ tightening is usually enough on its own. If a pocket sits below the jaw, it has to be moved, and only threads do that. If the pocket is there and the skin over it has gone slack too, Dr Nish plans both. Threads put the tissue back; Sofwave™ then firms the skin over its new position, as a staged plan, where clinically appropriate and in line with manufacturer guidance. He confirms which you are at the assessment. This is a guide, not the answer.
Filler does not lift a jowl, but it can make a jowl show less when the problem is partly a weak chin or a soft jaw edge. Jowls are tissue that has dropped or skin that has loosened; filler adds volume, so placed into the jowl itself it adds weight and can make it worse. Placed along the jawline or chin, where structure has receded, reversible filler gives the lifted tissue an edge to sit on and sharpens the line between face and neck. At Medisha, filler is added to a jowl plan only where Dr Nish judges it will change the result, usually alongside Sofwave™ or threads rather than instead of them.
Jowl treatment at Medisha Clinic, Mayfair, starts at £750. The single treatments: Sofwave™ jowls and jawline tightening is £1,000 for its one session, and a 720 PDO jowl lift is £750. The Lift & Tighten plan — the PDO jowl lift, then Sofwave™ jowls and jawline, in a sequence set by Dr Nish — is quoted after consultation. If the whole face needs work, Sofwave™ full face is £1,400 and a full face and neck thread lift is £1,995. Jawline or chin filler, where it earns its place, is £350 per ml. Follow-up reviews cost nothing extra. Dr Nish’s consultation is £50, redeemable in full against treatment. Medisha is at 58 South Molton Street, Mayfair, London W1K 5SL, 3 minutes on foot from Bond Street.
Jowls tend to show from the mid-40s, but the slackening behind them starts around 10 years earlier. The useful moment to act is when the jaw edge first looks soft in photos. At that stage Sofwave™ on its own usually does it, with no downtime, and may spare you a lift later. Once a pocket has settled below the jaw it needs threads: still non-surgical, still a result the same day. Nobody is too old. What matters is which of the two changes your jawline shows, and how much loose skin there is, not the date on your passport.
No — exercises, gua sha and face yoga do not remove jowls, because jowls are loosened skin and dropped tissue, not weak muscle. Working the muscles of the lower face does not tighten the skin over them or lift tissue that has slid down, and vigorous massage can make loose skin looser. They are harmless as habits, but they will not change a jowl. What changes a jowl is tightening the skin with Sofwave™, lifting the tissue with threads, or both, decided at consultation by a doctor who performs both.
Think of it as two timelines. Threads: the pocket is lifted the day they go in. The threads dissolve over 6 to 9 months and leave collagen behind. The lift holds for 12 to 18 months before a repeat is considered. Sofwave™: nothing to see on the day; the jaw edge starts to firm around week 12, keeps going to month 6, and is kept up with one session a year. Chin or jawline filler, where it is used, is reviewed at 12 to 18 months. Reviews with Dr Nish fall at 4 weeks (threads, filler) and 12 weeks (Sofwave™), and your upkeep plan is written at the second of them.
Jowls sit along the jawline, at the sides of the chin; a double chin sits under the chin, in the middle. Jowls are tissue that has slid down from the cheek and skin that has loosened over it. A double chin is usually fat under the chin, loose skin under the chin, or both. They often appear together, because the same loosening affects the whole lower face, and Dr Nish assesses the jawline and the neck as one. Jowls are treated with Sofwave™, threads or both; a double chin is treated according to whether it is fat, loose skin or dropped tissue.
Neither treatment is an ordeal, and you are back at your desk the same day or the next. Sofwave™ over the jowls takes about 30 minutes under numbing cream. It feels like pulses of deep warmth, and you can put make-up on and leave. Threads are placed under local anaesthetic. Expect the jaw to feel tender and look a little swollen for about 48 hours, sometimes with a bruise. For 2 weeks after threads: no heavy exercise, no massaging the jaw, no wide yawns or dental work, and 3 nights sleeping on your back. Filler, if used, means a few small injections and a few days of possible swelling. You go home with written aftercare and Dr Nish’s direct number.
Then you hear it at the assessment, in plain words, with a referral to a surgeon. Threads move tissue and ultrasound firms skin; neither removes skin. When loose skin runs from the jaw down the neck in a quantity no thread can gather, a facelift may achieve what these cannot. Dr Nish does not book a treatment that will not change your jawline. You leave with a clear picture of what surgery would involve and the facts to decide for yourself.
