Three questions decide what can be done for a face after weight loss: how much you lost, how fast, and how old you were when it came off. The fat that filled your face has gone, so skin that was stretched over it now hangs. Some of that skin will firm with Sofwave™. Some needs lifting back with PDO threads. A hollow cheek or temple may need structure put back. And some of it is excess that no non-surgical treatment can gather up, in which case Dr Nish Shah MD says so and refers you. Sizing that excess frankly is what the assessment is for.
1 · Tightens — Sofwave™
– Deflated look: skin slack but still roughly in place
– Firms the jawline, under-chin and neck; one visit, no downtime
– Suits a moderate loss, especially under 45
2 · Lifts — 720 PDO Threads
– Dropped look: a new jowl or a fold under the chin
– Tissue lifted back under local anaesthetic, visible on the day
– Holds 12–18 months; firmed with Sofwave™ later
3 · Restores — Structural Filler
– Hollowed cheekbone, temple or chin
– Reversible filler on the bone; structure, not fullness
– Only where it changes the result
Deflated, dropped, or more than either? How much came off and how fast tells Dr Nish most of it. The mirror tells him the rest.
Sized before it is sold. Dr Nish gauges how much excess there is and says what non-surgical treatment can and cannot take up
Three questions first. How much you lost, how fast, and your age at the time shape the whole plan
Deflated is not dropped. Slack skin is firmed with Sofwave™; a fold or jowl is lifted with threads
Structure, only where it changes the result. Reversible filler on the bone where the cheek or temple has emptied
Timed to your weight. Threads and filler wait until the scales have settled; tightening can start sooner
A surgeon when a surgeon is needed. Significant excess after a large loss is referred, not treated and hoped for

The Treatment
Why Does My Face Look Older After
Losing Weight?
Think of your face as a balloon with a certain amount of air in it. Let the air out quickly and the skin does not shrink to match. Facial fat sat under the skin like padding. Weight-loss injections and hard dieting remove it from the face as readily as from the waist. What is left is a skin envelope cut for a fuller face.
How much that envelope recovers depends on three things. How much you lost: a modest loss stretches the skin far less than a large one. How fast: skin keeps pace with a slow loss but not with a rapid one. And how old you were: skin in the 30s usually still has the elastin to spring back; skin in the 50s often does not. The result is a deflated look, slack skin that sits in place, or a dropped look, a jowl or a fold under the chin.
Then there is excess, and that is the conversation that matters most on this page. Sofwave™ firms slack skin and threads lift what has dropped, but neither removes skin. After a large, fast loss there may be more skin than any non-surgical treatment can gather up. Dr Nish sizes that in the mirror with you, and if surgery would achieve what these cannot, he tells you and refers you. Nothing on this page is sold to someone it will not help.
DEFLATED, DROPPED, HOLLOW — OR EXCESS
How Much Can Non-Surgical Treatment Take Up? The Frank Answer
Roughly this. Slack skin without a fold firms well. A fold or a jowl lifts well. Skin that hangs when you look down is excess, and excess needs a surgeon. Dr Nish performs the first 3 answers himself and is frank about the fourth.
- Deflated → Sofwave™. The jawline has softened and the neck is slack, but the cheeks still sit where they were. Ultrasound at about 1.5 mm makes the skin rebuild collagen over 12 weeks and firm to 6 months. One visit, no downtime, and it holds FDA clearance for lax skin under the chin and on the neck. It heats above the fat, and no fat loss was reported in its clinical studies. Works well after a moderate loss, particularly before 45.
- Dropped → 720 PDO threads. A jowl that was not there before, a fold under the chin, a cheek that has slid towards the mouth. Dissolvable cog threads lift the tissue back under local anaesthetic; you see the new position before you leave. The lift holds 12–18 months while the threads dissolve into collagen over 6–9. Usually after a larger loss, or at any size after 45.
- Hollowed → structure, only where it changes the result. Where the cheekbone, temple or chin has emptied, a lift alone leaves a gaunt face in a better position. Reversible filler placed deep on the bone gives the skin something to drape over, and often reduces how much lifting is needed. Dr Nish uses it sparingly and only where it alters the outcome.
- Two or three → a staged plan. Structure first, where needed, then threads, then Sofwave™ over the new position: separate visits, where clinically appropriate and in line with manufacturer guidance. Never combined on one day. Dr Nish sets the order and the gaps at consultation, and times the plan for when your weight has settled.
- Excess → a surgical opinion. Skin that hangs when you look down, or that you can gather in your hand, is more than tightening or threads can take up. Surgery may achieve what these cannot. Dr Nish says so at the assessment, refers you to a surgeon, and does not sell you a session on the way out.
Deflated or Dropped?
Two Faces in the Mirror
The deflated face — Sofwave™
— Lost the padding; kept its shape
The dropped face — 720 PDO threads
— Lost the padding; the shape followed
HOW MUCH CAME OFF?
Deflated, Dropped, Hollow
— or Excess?
The same weight loss leaves different faces. Here is how Dr Nish sorts them, and what he tells each.
Deflated — Sofwave™
- Skin slack, nothing has moved
- A moderate loss, taken slowly, usually under 45
- One visit; firms over 6 months
- Often the whole plan
Dropped — PDO Threads
- A jowl or under-chin fold that is new
- A larger or faster loss, or any loss after 45
- Lifted on the day; lasts 12–18 months
- Sofwave™ tightens over it at a later visit
Hollow — Structure
- Cheekbone, temple or chin emptied
- Common after a large loss at any age
- Reversible filler on the bone, one visit
- Only where it changes the result
Excess — Surgery
- Skin you can gather in your hand
- Very large or very rapid loss
- What these cannot take up, a surgeon can
- Dr Nish says so early, and refers you
WHAT GOES WHERE
What Medisha Can Do After Weight Loss
— and What It Sends to a Surgeon
Six possible parts of a plan. Dr Nish performs the first 5 himself; the sixth is a conversation and a referral.
Sofwave™ Face & Neck
For the deflated face. Firms the slack jawline and neck in one visit; nothing to recover from.
720 PDO Jowl Lift
For the dropped face. A new jowl or under-chin fold lifted back on the day; holds 12–18 months.
Dermal Fillers — Cheek, Temple, Chin
Structure, only where it changes the result. Reversible filler on the bone where the face has emptied.
Non-Surgical Facelift — Staged Plan
Dropped and deflated together. Threads at one visit, Sofwave™ at another, in the order Dr Nish sets.
Non-Surgical Neck Lift
When the neck took the brunt: a slack or folded neck treated on its own terms.
Surgical Referral
When the excess can be gathered in your hand. Dr Nish explains why, and who to see.
Suitability
Can My Face Be Treated Without Surgery After Weight Loss?
After a moderate loss, usually yes. After a very large or very rapid loss, sometimes partly, and sizing the excess frankly is the first thing Dr Nish does. Weight that has settled matters more than the number on the scales.
Good Candidates
- A jawline and neck that went slack as the weight came off
- A jowl or a fold under the chin that was not there before
- A cheekbone, temple or under-eye that has emptied
- Looking older or gaunt in photographs since the loss
- Weight settled, or within sight of target
- Preferring staged non-surgical visits to an operation
- Men and women alike; no upper age limit; every skin tone
May Not Be Suitable
- Excess you can gather in your hand — only surgery removes skin; Dr Nish refers you
- Weight still falling fast — threads and filler wait until it has settled
- Skin so thin over so little fat that a thread might show through
- An infection on the skin where treatment would go
- Pregnant or breastfeeding at the time
- A pacemaker or metal implant in the treatment zone, for Sofwave™
Excess is the deciding factor, and it cannot be judged from a photograph or a video call. Dr Nish examines the face in person, in profile and looking down, asks how the weight came off, and tells you what is realistic. The £50 consultation is redeemable in full against treatment. There is no obligation to go ahead, and no pressure to.

Loose Skin After Weight Loss
— Answered by Dr Nish Shah
Medically written and reviewed by Dr Nish Shah MD — Medical Director, Medisha Clinic · Senior International Clinical Trainer, Derma Medical · Official certified Sofwave™ practitioner and partner · Brand Ambassador & KOL for Intraline (PDO threads & dermal fillers) · Last reviewed 9 October 2026
“Ozempic face” is a nickname, not a diagnosis. It describes a face that looks older after fast weight loss, including weight loss with GLP-1 injections. The signs are slack skin at the jaw and neck, a new jowl or under-chin fold, and hollows at the cheekbone and temple. The medicine does not do this to the skin. The speed and size of the fat loss do, by removing the padding the skin was stretched over. A crash diet that shifts the same weight at the same speed leaves the same face. Dr Nish Shah MD sorts the deflated from the dropped and the hollow, and sizes any excess.
Often, and the answer depends on how much excess there is rather than on how much weight you lost. Slack skin that still sits in place firms with Sofwave™, which holds FDA clearance for lax skin under the chin and on the neck. One visit, no downtime, collagen rebuilt over 12 weeks and firming to 6 months. A fold or jowl that has formed is lifted back with a PDO thread lift. Skin that hangs when you look down, or that you can gather in your hand, is excess; non-surgical treatment firms and lifts but does not remove skin. In that case an operation may achieve what these treatments cannot, and Dr Nish tells you so and refers you.
Start with the mirror and three questions: how much did you lose, how fast, and how old were you at the time. A moderate loss, taken slowly, before 45 usually leaves a deflated face, slack but in place, which is Sofwave™ territory. A larger or faster loss, or a loss after 45, more often leaves a dropped face, with a jowl or fold that only threads move back. If the cheekbone or temple has emptied, reversible filler on the bone puts structure back, and only where it changes the result. Where two apply, Dr Nish stages them, structure, then lift, then tightening, at separate visits.
For threads and filler, yes: both are placed to a shape, and a face still losing fat changes shape under them. Dr Nish usually asks for a settled weight, or one within sight of target, before lifting or restoring. Sofwave™ is the exception. It works by making the skin rebuild collagen rather than by moving tissue. So it can be started while the weight is still coming off, to help the skin keep pace. If you are on weight-loss injections, the plan is drawn around where your weight is heading, and reviewed as it gets there.
At Medisha Clinic in Mayfair, Sofwave™ for the full face and neck is £1,800. A 720 PDO thread lift is £750 for one area, or £1,995 for the full face and neck. Structural filler is £350 per ml. Staged plans that combine threads with Sofwave™, the Lift & Tighten plan and the Complete Non-Surgical Facelift, are sequenced by Dr Nish and quoted after consultation. Reviews are included. The £50 consultation with Dr Nish is redeemable in full against whatever treatment follows. Medisha is at 58 South Molton Street, Mayfair, W1K 5SL, near Bond Street station.
Some of it may, and the three questions predict how much. Skin in the 20s and 30s, after a moderate loss, often recovers a good deal over 6 to 12 months once the weight settles. Skin that was stretched for years, or that lost its padding quickly after 40, usually does not come all the way back. Collagen is no longer made at the rate needed. Dr Nish’s usual advice is to give it 6 months at a settled weight, then treat what remains. Sofwave™ is the first answer for what remains slack, because it makes the skin rebuild collagen it would not otherwise produce.
Not when it is used as structure rather than as stuffing. A face that has lost weight has emptied at the cheekbone, temple and chin. A small amount of reversible filler, placed deep on the bone there, restores a frame for the skin to sit on. The overfilled look comes from filler pushed into skin that has dropped, which adds weight to tissue that needs lifting. At Medisha, filler is used only where it changes the result, threads do the lifting and Sofwave™ does the firming. Dr Nish will tell you whether structure is part of your plan or not.
Yes, because Sofwave™ does not care why the skin loosened. It treats the slackness itself by making the skin rebuild collagen at about 1.5 mm, and it heats above the fat, with no fat loss reported in its clinical studies. The jawline, under-chin and neck are where a face on weight-loss injections slackens first, and Sofwave™ holds FDA clearance to lift lax tissue under the chin and on the neck. What it cannot do is lift a jowl that has formed or refill a hollow. Where those are present it is one stage of a plan, not the plan. It is also the one treatment Dr Nish may start before your weight has fully settled.
As long as your weight holds, broadly speaking. A PDO thread lift holds 12 to 18 months; the threads themselves are gone within 6 to 9 months, leaving collagen in their place. Reversible filler lasts 12 to 18 months. Sofwave™ builds collagen that is yours to keep, with a yearly visit to stay ahead. The caveat is weight: another large loss empties the face again and may call for a further plan, and a large gain stretches it again. Dr Nish reviews you at 4 weeks after threads or filler and 12 weeks after Sofwave™, and maps out what comes next.
Less than most people expect, and it differs by treatment. Sofwave™ under numbing cream feels like pulses of deep warmth; you can go straight back to work. Filler is placed under local anaesthetic and may leave a few days of swelling. Threads are the one to plan around. Expect local anaesthetic, about 48 hours of swelling and tenderness, perhaps a bruise, then 2 weeks without heavy exercise, massage, wide yawning or dental work. Sleep on your back for 3 nights. Written aftercare and Dr Nish’s direct number go home with you.
Then excess, not slackness, is likely to be the main problem, and this is the frank conversation at the heart of this page. Tightening firms skin and threads reposition it, but no non-surgical treatment removes skin. If you can gather the skin at your jaw or neck in your hand, or it hangs when you look down, surgery may achieve what these treatments cannot. Dr Nish will say so at the assessment and refer you to a surgeon. Sometimes the answer is split: a surgical opinion for the neck, say, and non-surgical treatment for the face.
Because the question after weight loss is not which treatment but how much can be done. That needs a doctor who performs all the options and gains nothing by picking one. Dr Nish Shah MD performs Sofwave™ as an official partner clinic, PDO thread lifts with thousands behind him, and structural filler, and stages them himself. At Derma Medical, one of the UK’s largest aesthetics training academies, he is the Senior International Clinical Trainer and teaches thread lifting. He is also Brand Ambassador and Key Opinion Leader for PDO threads and dermal fillers.
