CREPEY TEXTURE · 1. SOFWAVE™ FIRMS · 2. THREADS LIFT · 3. PROFHILO THICKENS — DR NISH DECIDES

Loose or Crepey Skin – Tighten with Sofwave™, Lift with PDO Threads, Rebuild with Profhilo. Assessed by Dr Nish Shah MD, Mayfair

Pinch the skin on your neck, the back of your hand or your knee, and watch it settle. If it crinkles like tissue paper before it smooths, that is crepey skin. It is a texture problem, not a sagging problem: the collagen and elastin that once snapped it flat have thinned, and no cream reaches the layer where they are made. Sofwave™ does, in one session with no downtime, and the skin rebuilds its own collagen over 6 months. Profhilo or Sunekos Performa thickens skin that has gone papery; PDO threads lift anything that has slipped as well. Dr Nish Shah MD examines the texture in person and tells you which applies.

Is it texture, or has something slipped? Crepey skin is usually texture alone. The pinch test at your assessment settles it.

The pinch test comes first. Dr Nish pinches, releases and watches the recoil before any treatment is named

Texture is treated at depth. Sofwave™ reaches about 1.5 mm, where collagen is built, without breaking the surface

6 months of improvement from one visit. Firmer at 12 weeks, still firming at 6 months, as your own collagen returns

Nothing to recover from. Sofwave™ leaves no marks; a skin booster leaves small bumps for a day

Plain words about excess. Skin that hangs in folds needs a surgeon, and Dr Nish will tell you so

Every answer, one pair of hands. Official certified Sofwave™ partner clinic; Official UK Trainer for Sunekos; thousands of thread lifts

The Treatment

Why Has My Skin Gone Loose and Crepey?

Crepey skin is skin that has lost its spring. Two fibres hold skin flat: collagen for firmness, elastin for recoil. From the mid-30s the body makes a little less of each every year. Sun, smoking, the menopause and a stretch of weight loss hurry it along. The skin thins, creases on itself and stays creased a moment too long.

Where does it show? Wherever the skin is thinnest and most moved. The neck and the décolletage, where you notice it on waking. The eyelids and under the eyes. The inner arm, the knee, the back of the hand. On the face it is the fine crinkling that appears when you lean forward over a mirror.

A dropped jowl or a hood over the eye is a different problem: tissue has slid, and only a lift moves it back. Crepey texture needs the skin itself rebuilt. Dr Nish pinches the skin, watches the recoil and tells you which you have. If both, he stages the plan. If the skin hangs in folds rather than crinkles, he says so: surgery may achieve what no non-surgical treatment can, and he refers you.

WHAT WORKS ON TEXTURE

Why Moisturiser Cannot Fix It
— and What Can

Moisturiser sits in the outermost 0.1 mm of skin and plumps it with water for a few hours. Crepey texture starts about 1.5 mm down, where collagen is built. Each treatment below reaches that depth in a different way, and Dr Nish performs all of them.

  • Crinkled, nothing dropped → Sofwave™. Parallel-beam ultrasound heats a line of points at about 1.5 mm while SofCool™ keeps the surface cool. The heated collagen contracts, then the skin lays down new fibres over 12 weeks. In the pivotal study, 88% had improved at 12 weeks from one session. No downtime, any skin tone, and the texture keeps firming to 6 months.
  • Crepey and papery-thin → Profhilo or Sunekos Performa. When the skin has thinned so far that it creases at rest, tightening alone leaves it papery. A course of small injections, 2–4 visits, restores hydration, elasticity and thickness. Neither lifts. Dr Nish adds them after the skin has been tightened.
  • Crinkled and slipped → 720 PDO threads. Where the jowl, cheek or under-chin has slid down as well, tightening alone leaves a fold. Dissolvable cog threads reposition the tissue under local anaesthetic, with the lift visible before you leave. They dissolve over 6–9 months and the lift holds 12–18. Sofwave™ then firms the texture at a later visit.
  • More than one → staged, never same-day. Threads and Sofwave™ are separate visits in a staged plan, where clinically appropriate and in line with manufacturer guidance. A booster comes last. Dr Nish sets the sequence and the gaps at consultation.
  • Folds, not crinkles → a surgical opinion. Non-surgical treatment firms and repositions skin; it cannot cut any away. When the excess hangs rather than crinkles, Dr Nish says that surgery may achieve what these cannot, and refers you to a surgeon.

Sofwave™ or a Skin Booster
— What You Notice, and When

1. Sofwave™ — Firms the texture

— Crinkled skin that still sits in place

At 6 weeks Little to see yet; collagen is forming beneath
At 12 weeks Skin pinches smoother and recoils faster
At 6 months Full result; the crinkle has softened or gone
Visits One, 30–60 minutes under numbing cream
Marks afterwards None; make-up straight back on
Keep it going One visit a year, in Dr Nish’s experience

3. Profhilo / Sunekos — Thickens thin skin

— Papery skin that creases at rest

At 6 weeks After the second visit, skin usually feels plumper and more hydrated
At 12 weeks Fine creasing softer; glow returns
At 6 months Fading; a repeat course keeps it
Visits Two (Profhilo) or four (Sunekos), spaced as the product requires
Marks afterwards Small bumps at the injection points for a day
Keep it going A course every 6–9 months

THE PINCH TEST

Crinkles, Thinned, Slipped
— or Hanging in Folds?

Pinch the skin on your cheek, your neck or your arm, let go, and watch. What it does next tells Dr Nish most of what he needs.

Crinkles, then settles — Sofwave™

  • Fine tissue-paper lines that smooth out within a second
  • Nothing has moved; the sheet has simply thinned
  • Firms from 12 weeks to 6 months
  • Usually the whole answer

Creases at rest — Profhilo / Sunekos

  • Papery, dull, dehydrated even without pinching
  • Tightening alone leaves it thin
  • A course of 2–4 visits
  • Added once Sofwave™ has firmed it

Slipped as well — PDO Threads

  • A jowl, hood or under-chin fold that pinching does not explain
  • Repositioned on the day
  • Threads, then Sofwave™, sequenced by Dr Nish
  • Sofwave™ firms the texture at a later visit

Hangs in folds — Surgery

  • Excess that stays folded when you let go
  • Surgery may achieve what tightening cannot
  • Dr Nish tells you so, and refers you
  • You are not sold a session that will not show

Suitability

Can My Loose Skin Be Tightened
Without Surgery?

Crepey texture without excess responds well, and that describes most people who ask. The questions are how thin the skin has become and whether anything has slipped with it.

Good Candidates

  • Skin that crinkles like tissue paper when pinched or when you lean forward
  • A neck or décolletage that looks lined on waking and smooths by lunchtime
  • Crepey eyelids, inner arms, knees or the backs of the hands
  • Texture that moisturiser plumps for an hour and no longer
  • Skin thinned by sun, the menopause or a stretch of weight loss
  • Preferring one visit and no marks to a course of needles
  • Any decade from the late 30s; any skin tone

May Not Be Suitable

  • Skin hanging in folds — surgery may achieve what tightening cannot; Dr Nish refers you
  • A pacemaker or metal implant under the area to be treated with Sofwave™
  • Broken or infected skin where the treatment would go
  • Pregnancy, or while breastfeeding
  • Blood thinners, if threads are part of the plan, until reviewed
  • Hoping a booster will lift a jowl — it thickens skin; it cannot move tissue

Most people who worry about crepey skin have texture loss alone, and that is the easier problem. Dr Nish confirms it by examining the skin, not from a photograph. The £50 assessment is redeemable in full against treatment.

For crepey skin that has thinned but not dropped, Sofwave™ is the treatment Dr Nish reaches for first at Medisha. It is ultrasound, delivered about 1.5 mm down where collagen is built, in one session with no downtime. In the pivotal study 88% of patients had improved at 12 weeks, and the skin goes on firming to 6 months. It holds FDA clearance to lift the brow and lax under-chin and neck skin. Where the skin is so thin it creases at rest, Profhilo or Sunekos Performa thickens it afterwards. Where a jowl or fold has slipped, only a PDO thread lift moves it back.

Crepey skin comes from losing the two fibres that keep skin flat: collagen, which gives firmness, and elastin, which gives recoil. Both decline from the mid-30s, and faster with sun exposure, smoking, the menopause or losing weight. Thinner skin creases on itself and recovers more slowly, so you see fine tissue-paper lines on the neck, chest, eyelids and inner arms. It is a change in the skin itself. A face that has dropped is a different change, in the deeper supporting tissue, and needs lifting rather than firming.

Pinch the skin and let go: the answer is usually in what happens next. If it crinkles and then settles, the texture has thinned but nothing has moved, and Sofwave™ alone is usually enough. If it looks papery and creased before you pinch it, the skin needs thickening too, with Profhilo or Sunekos Performa once it has been tightened. If a jowl, hood or under-chin fold stays put when the skin is lifted, tissue has slipped, and a PDO thread lift repositions it in a staged plan. Dr Nish does this test with you in the mirror.

Sofwave™ at Medisha Clinic, Mayfair, is priced by area. The brow is £700 and the jowls and jawline £1,000. The full face is £1,400, face with neck £1,800, and face, neck and brow £2,200. A PDO thread lift, where tissue has slipped, is £750 per area. Profhilo is £350 a session (2 sessions £700) and Sunekos Performa £350 a session (4 sessions £1,100). A staged plan of a PDO lower-face lift followed by Sofwave™ face and neck is sequenced by Dr Nish and quoted after consultation. Review appointments are included. Consultation with Dr Nish costs £50, redeemable in full against treatment. Medisha is at 58 South Molton Street, W1K 5SL, a 3-minute walk from Bond Street.

Yes, within the limits of what ultrasound can do: it firms skin that has thinned, and it does not move tissue that has dropped. The FDA cleared Sofwave™ in 2021 to lift the eyebrow and lax under-chin and neck tissue, on the strength of a five-site clinical study. In the pivotal study published in Dermatologic Surgery, 88% of patients had improved at 12 weeks after one treatment. You notice little at 6 weeks, a firmer pinch at 12 weeks, and the full change at 6 months. One session of 30 to 60 minutes, and one a year to maintain it.

No, and the reason comes down to depth. Moisturiser works in the outermost tenth of a millimetre, drawing in water that plumps the crinkle for a few hours. Crepey texture is a loss of collagen and elastin about 1.5 mm down, and nothing rubbed on the surface reaches that layer. Collagen drinks are digested like any other protein. Facials polish the surface. Retinoids and daily SPF are worth keeping because they slow further loss. What rebuilds collagen at depth is Sofwave™; what thickens thinned skin is an injected booster; what moves a dropped fold is threads.

Sofwave™ feels like pulses of deep warmth under numbing cream, and the cooling tip keeps the surface comfortable. You can go to a meeting afterwards; there is nothing to see. Profhilo and Sunekos are a few small injections, with bumps at the points for a day. PDO threads are placed under local anaesthetic; expect 48 hours of swelling and tenderness, possibly a bruise, and a fortnight of taking care. Written aftercare and Dr Nish’s own number come home with you.

Yes, Sofwave™ is used on every skin tone, including the deepest. It is ultrasound, so it has no interest in pigment; the energy is absorbed as heat at depth, not by melanin at the surface. That is why it suits the deeper tones where light and laser devices carry a risk of dark or pale patches. SofCool™ cooling protects the surface throughout. Profhilo, Sunekos and PDO threads are likewise used on all skin tones. Dr Nish treats a wide range of skin at Medisha and will discuss your own at the assessment.

Most people first notice crepey texture between the late 30s and mid-40s, on the neck or around the eyes, and that is a sensible time to act. Firming the skin while it is only thinned is easier than lifting it once it has slipped. There is no upper age limit. What matters is the state of the skin, not the date of birth. The one exception is excess: at any age, skin that hangs in folds may need a surgeon, and Dr Nish will say so.

Think in checkpoints rather than an expiry date. After Sofwave™, your skin is firmer at 12 weeks and firmer still at 6 months, and that collagen is yours to keep. A yearly session keeps ahead of the ageing that continues underneath. A PDO thread lift holds for 12 to 18 months, with the threads dissolving over 6 to 9 months and leaving collagen behind. Profhilo and Sunekos Performa fade over 6–9 months and are repeated. Dr Nish reviews Sofwave™ at 12 weeks, and threads or boosters at 4 weeks, then plans the next step with you.

Then you hear it at the assessment, before any money changes hands. Sofwave™ firms skin and threads reposition tissue, but neither removes a single centimetre of it. If the skin hangs in folds when you let go rather than crinkling and settling, an operation may achieve what firming and lifting cannot. Dr Nish says so plainly and refers you to a surgeon. Medisha’s rule is simple: no session is sold that will not show in the mirror.

Because crepey skin is assessed and treated here by one doctor, Dr Nish Shah MD, who performs every option himself. Sofwave™, as an official partner clinic. Profhilo and Sunekos Performa, as the Official UK Trainer for Sunekos. PDO threads, with thousands of thread lifts behind him. He teaches thread lifting to other doctors as Senior International Clinical Trainer at Derma Medical, and is Brand Ambassador and Key Opinion Leader for PDO threads. He has performed over 30,000 non-surgical treatments and is one of the most experienced aesthetic doctors in the UK and London. If a surgeon would serve you better, he says so. Medisha is rated 5.0 on Google from 128 reviews, in Mayfair and Marylebone.
